Exercise Recommendations For Multiple Sclerosis Patients With Heat Intolerance: A Narrative Review

Abstract
Multiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system (CNS). About 80% of MS patients show a transient increase in clinical signs and symptoms due to the core temperature increases. Regular exercise has been recommended as its beneficial effects have been proven but as it is known exercise increases core temperature and it limits exercise capacity. Although there have been many reviews and metaanalyses on the various aspects of exercise training, there are only a few reviews on the thermal sensitivity due to exercise training in MS patients. The purpose of this narrative review is to describe causes of heat intolerance in MS patients, effect of exercise on core temperature and to discuss how to design an optimal exercise program with taking into account the factors that increases core temperature.

Multipl skleroz (MS), merkezi sinir sisteminde (MSS) kronik inflamatuar bir hastalığıdır. MS hastalarının yaklaşık % 80'inin klinik belirti ve bulguları çekirdek sıcaklıktaki artışa bağlı olarak geçici bir artış göstermektedir. Egzersizin yararları kanıtlanmıştır ve düzenli egzersiz her zaman tavsiye edilmektedir. Ancak egzersizin çekirdek sıcaklığı artırdığı, artan bu sıcaklığın da egzersiz kapasitesini limitlediği bilinmektedir. Egzersiz eğitiminin özellikleriyle ilgili birçok değerlendirme ve meta-analiz olmasına rağmen MS hastalarında egzersiz eğitimi nedeniyle oluşan sıcaklığa duyarlılık hakkında sadece birkaç derleme bulunmaktadır. Bu derlemenin amacı, MS hastalarında ısı hassasiyetinin nedenlerini ve egzersizin çekirdek sıcaklığa etkisini açıklamak, çekirdek sıcaklığını artıran faktörleri göz önünde bulundurarak optimal bir egzersiz programın nasıl oluşturulacağını tartışmaktır.
