Urinary calcium levels in urolithiasis patients and importance of Pak test

Abstract
The aim of this study is to find out the patients with latent hypercalsuria and urolithiasis with normal calcium output and heretofore to indentify the group of the patients that will be candidates of preventive therapy . Hypercalsuria was detected in 17 of 48 patients who were evaluated metabolically due to urolithiasis between January 1994 and January 1995. The diet programme (low Calcium and Sodium) was performed in all of the patients. Then, the urine sample was collected after two hours of fasting and after four hours of 1 gr Calcium intake. Afterwords, The Pak test was performed in all of the patients. Absorbtive type 2 hypercalsuria was detected in two of the patients who had normocalsemia with normal diet programme. Fi¬nally, these results suggest that all of the patients with urolithiasis must be evaluated with Pak test to detect latent hypercalcuria.
Özet
Çalışmanın amacı idrarlarında normal kalsiyum değerleri olan ürolitiyazisli olgularda gizli hiperkalsürinin olup olmadığını tespit etmek ve böylece taş oluşumunu önleyici tedavi uygulanacak grubu belirlemektir. Ocak 1994 -Ocak 1995 tarihleri arasında üriner sistem taş hastalığı nedeniyle metabolik değerlendirme yapılan 48 olgunun 17'sinde (% 35.6) hiperkalsiüri tespit edildi. Hastaların tümü kısıtlı kalsiyum ve sodyum diyeti' uygulandıktan sonra, iki saatlik açlık ve dört saatlik 1 gr kalsiyum yüklemesi sonrası idrar örnekleri alınarak Pak'ın ayırıcı tanı testine tabi tutulmuştur. Normal diyet altında normokalsemik olan iki olguda ayırıcı test sonrası absorbtif tip 2 hiperkalsiüri tespit edilmiştir. Bu sonuçlar gizli hiperkalsiürinin ortaya çıkarılması için tüm üriner sistem taşlı olgulara Pak ayırıcı tanı testinin yapılması gerektiğini düşündürmektedir.
