The role and importance of mediastionscopy in diagnosis and staging of the lung cancer

Abstract
Mediastinoscopy was performed on 21 patients to investigate its role in the diagnosis and staging of lung cancers between January 1994 and January 1997. Mediastinoscopy was carried out under general anesthesia in the supine position according to Carlen's method. Of the patients, 11 (52.3 %) yielded positive results; among these 8 were epidermoid carcinoma, 2 small cell carcinoma metastasis and one was bronchoalveolar carcinoma. As complications, pneumothorax was present in 1 case, minimal mediastinal bleeding in 2 cases and wound infection in one case. There was no associated mortality. Mediastinoscopy had an important role in the correct diagnosis and management of lung cancers and is crucial to prevent unnecessary thoracotomies. Therefore, it must be performed as a routine investigation.

Ocak 1994 -Ocak 1997 tarihlari arasında akciğer kanserlerinin tanı ve evrelendirilmesinde mediastinoskopinin rolü araştırılmak üzere 21 hastaya mediastinoskopi yapıldı. Mediastinoskopi teknik olarak genel anestezi altında, sırt üstü pozisyonda ve Carlens'in tarif ettiği klasik metodla yapılmıştır. Çalışmaya alınan 21 hastanın 11 'inde pozitif sonuç (% 52.3) elde edilmiş olup, bunların sekizi epidermoid karsinom, ikisi küçük hücreli karsinom metastazı, bir hasta ise bronkoalveoler karsinom tanısı almıştır. Yirmibir mediastinoskopinin birinde pnömotoraks ikisinde minimal mediastinal kanama birinde de yara enfeksiyonu oluşmuş, mortalite görülmemiştir. Mediastinoskopi akciğer kanserli hastalarda gereksiz torakotomiyi önlemek ve kesin tanı ile birlikte tedavi biçimini yönlendirmekte önemli bir rol oynamaktadır. Bu nedenle mediastinoskopi rutin uygulama alcım içine konmalıdır. .
