Determination of fetal hemoglobin levels of anemic patients in different age groups by alkali denaturation method

Abstract

In this study, fetal hemoglobin levels (HbF) of 48 subjects aged between 2 to 15 years as well as of 80 adults were determined by alkali denaturation method. The pediatric group consisted of 30 anemic (20 M, 10 F) and 18 healthy subjects (10 M, 8 F). Adult group consisted of 60 anemic (42 F, 18 M) and 20 healthy subjects (10 M, 10 F). In addition, HbF levels of 85 anemic patients with mean corpuscular volume (MCV) lower than 78 fl (59 adults, 43 F, 16 M; 3-15 ages 16M, 10F) and of the control group 20 adults (10 F, 10 M) and 18 subjects aged between 2-15 years (10 M and 8 F) were also determined. A positive correlation between HbF % and MCV were investigated in all groups. Significant correlations were observed for anemic females, anemic patients aged 2-15 and the group with MCV 78 fl. Statistically significant correlation was not observed for adult male anemic patients, adult controls and the control group with MCV 78 fl . A significant correlation was also observed for the 2-15 aged control group. It is concluded that the reason of anemia was not related to HbF since all of the examined groups did not have pathologic HbF levels. However, alkali denaturation is an easy, cheap and reproducible method and can be recommended for HbF % estimation.

Bu çalışmada, 2-15 yaş grubunda 48 ve erişkin 80 bireyin fetal hemoglobin (HbF) düzeyleri alkali denatürasyon yöntemi kullanılarak saptanmıştır. İki-15 yaş grubunda 30 anemili hasta (20 E, 10 K) ve kontrol grubunda (10 E, 8 K), erişkin anemili hasta grubunda ise (42 K, 18 E) kontrol grubunda ise (10 E, 10 K) yer almıştır. Ayrıca MCV 78 fl den düşük olan 85 anemili hastanın (erişkin, 43 K, 16 E; 3-15 yaş 16 E, 10 K) ve kontrol grubunun da (erişkin 10 K, 10 E; 2-15 yaş 10 E, 8 K) HbF düzeyleri saptanarak, tüm gruplarda HbF ile MCV arasındaki ilişki incelenmiştir. Anemili kadın, 2-15 yaş anemik hasta grubu ile MCV 78 fl olan hastalarda MCV ile HbF arasında anlamlı ilişki saptanmıştır. Yetişkin anemili erkek ile yetişkin kadın ve erkek kontrol grubu ve MCV 78 fl olan kontrol grubunda bu ilişki bulunamamıştır. İki-15 yaş kontrol grubunda ise anlamlı ilişki bulunmuştur. İncelenen gruplarda patolojik değerde HbF bulunmaması nedeni ile aneminin HbF'ne bağlı olmadığı düşünülmüştür. Bunun yanısıra alkali denatürasyon yöntemi kolay, ucuz ve tekrarlanabilir olduğundan, % HbF saptamalarında kullanılabilecek bir yöntem olduğu sonucuna da varılmıştır.
