Abstract

Acute hemorrhagic edema of infancy is an acute form of cutaneous leukocytoclastic vasculitis. Hemorrhagic purpura and inflammatory edema of the limbs and face are two main features and visceral involvement is uncommon. Spontaneous and complete resolution occurs within one to three weeks; one to four attacks may occur. A ten- month - old girl was admitted with a history of seven days of rash on the face and on extremities which started after measles vaccination during a respiratory tract infection. The rash progressed to widespread palpabl hemorrhagic purpura within a few hours. Histopathological examination of the skin revealed leukocytoclastic vasculitis. On the seventh day of admission to the hospital the lesions spontaneosly improved without therapy. The case was presented to emphasize that acute infantile hemorrhagic edema must be considered in all infants with purpuric rash and that the disease could be confused with meningococsemia purpura fulminans and Henoch Schönlein purpura.

Özet

Akut infantil hemorajik ödem, çok nadir olmayan fakat iyi tanınmayan bir hastalıktır. Klinik olarak süt çocuklarında sıklıkla yanaklarda, kulak kepçelerinde, ekstremitelerde beliren hemorajik purpura ve ödemle karakterizedir. Hastalığın prognozunun iyi olduğu ve genellikle 1-3 hafta içinde kendiliğinden tam şifa ile iyileştiği bildirilmektedir. Bu yazıda sunulan on aylık kız hastada; kızamık aşısı ve solunum yolu infeksiyonu sonrasında, yüzünde, kol ve bacaklarında başlayan küçük kırmızı döküntüler, saatler içinde yaygın palpabl hemorajik purpuraya dönüştü. Cilt biyopsisinde lökositoklastik vaskülit tesbit edildi ve döküntüler bir haftada tamamen düzeldi. Döküntüler özellikle meningokoksemi, purpura fulminans ve Henoch-Schönlein purpurası ile karıştırılabileceğinden hastalığın daha iyi tanınması gerektiğini vurgulamak amacıyla sunuldu.
