Effects of intravitreal trovofloxacin on vitreous nitric oxide levels in experimental Staphylococcus epidermidis endophthalmitis

Abstract
Purpose: This study was designed to test the effects of intravitreal trovafloxacin on the vitreous nitric oxide levels on an experimental rabbit model of Staphylococcus epidermidis (S. epidermidis) endophthalmitis. Material and Methods: Among twenty-six rabbits, 22 were given intravitreal 100 000 CFU S. epidermidis ATCC 12228. At 24 hours, Group 1 (8 rabbits) and at 48 hours, Group 2 (8 rabbits) received 100 mg intravitreal trovafloxacin. Group 3 (6 rabbits) was used as inoculated but untreated controls. Four rabbits (Group 4) were used as uninfected controls. Clinical scores were calculated at 24, 48 and 72 hours. Microbiological and histopathological scorings were made. Vitreous nitric oxide levels were determined in challenged animals and in controls and were correlated to the microbiological and histopathological scores. Results: Median vitreous nitric oxide levels were as follows: Group 1 Right eyes: 208.5085 mmol/L, Left eyes:148.6170 mmol/L (p=0.036); Group 2 Right eyes: 254.9505 mmol/L, Left eyes: 220.7920 mmol/L (p=0.735); Group 3 Right eyes: 217.8220 mmol/L, Left eyes: 165.8090 mmol/L (p=0.068); Control Right eyes: 132.6530 mmol/L, Left eyes: 189.1400 mmol/L (p=0.180; Wilcoxon signed ranks test). Vitreous nitric oxide levels did not correlate with the clinical and histological scores in any of the groups. (p 0.05). In group 1 right eyes, there was a negative correlation between the vitreous nitric oxide levels and the microbiological scores. (r=-0.916; p=0.001; Spearman' s correlation analysis). Conclusion: Observed increase in the vitreous nitric oxide levels despite a decrease in the number of cultured microorganism may suggest that nitric oxide has an inhibitory effect on the growth of S. epidermidis. These are the preliminary results and we conclude that peroxynitrite levels must also be determined to better evaluate the role of nitric oxide in experimental endophthalmitis models.

Amaç: Bu çalışmada deneysel tavşan modelinde oluşturulan Staphylococcus epidermidis (S.epidermidis) endoftalmisinde intravitreal trovafloksasinin vitreus nitrik oksit düzeylerine etkisi araştırılmıştır. Gereç ve yöntem: Çalışmada 26 tavşan kullanıldı ve 4 gruba ayrıldı. İlk üç gruptaki 22 tavşanın sağ gözüne deneyin birinci günü intravitreal 100000 CFU S.epidermidis ATCC 12228 suşu, dördüncü gruptaki 4 tavşanın sağ gözlerine ise serum fizyolojik enjekte edildi. Enjeksiyonu takib eden 24. saatte birinci gruptaki 8 tavşanın sağ gözüne, 48. saatte ikinci gruptaki 8 tavşanın sağ gözüne intravitreal 100mg trovafloksasin uygulandı. Üçüncü grubu oluşturan 6 tavşanın sağ gözü, tedavi uygulanmayan kontrol grubunu oluşturdu. Yetmişikinci saatte tüm gözler enüklee edildi ve klinik, histolojik, mikrobiyolojik ve biyokimyasal olarak değerlendirildi. Bulgular: Vitreus nitrik oksit (ortanca) düzeyleri sağ ve sol gözlerde şöyle bulundu: Grup 1 Sağ: 208.5085 mmol/L, Sol: 148.6170 mmol/L (p=0.036); Grup 2 Sağ: 254.9505 mmol/L, Sol: 220.7920 mmol/L (p=0.735); Grup 3 Sağ: 217.8220 mmol/L, Sol: 165.8090 mmol/L (p=0.068); Grup 4 (kontrol) Sağ: 132.6530 mmol/L, Sol: 189.1400 mmol/L (p=0.180; Wilcoxon signed ranks testi). Gruplarda vitreus nitrik oksit düzeyleri ile klinik ve histolojik skorlar arasında anlamlı bir korelasyon saptanmadı (p 0.05). Grup 1'de vitreus nitrik oksit düzeyleri ve mikrobiyolojik skorlar arasında negatif korelasyon saptandı (r=-0.916; p=0.001; Spearman korelasyon analizi). Sonuç: Vitreus nitrik oksit düzeyinin artması ile mikrobiyolojik skorların azalması, nitrik oksitin mikroorganizmanın çoğalmasını önleyici yönde etki yaptığını düşündürmektedir. Bunlar ilk sonuçlardır ve endoftalmi modellerinde vitreus nitrik oksit düzeylerine yönelik çalışmalarda peroksit düzeylerinin de saptanması, gerçek hasar hakkında bize daha ayrıntılı bilgi verebilecektir.
