Effects of some factors on dehydration in children with diarrhoea

Abstract
Purpose: This investigation was carried out in order to determine the effects of some social, demographic and medical factors on dehydration in children with diarrhoea. Materials and Methods: The investigation was carried out on 164 0-5 year-old children who were brought to the general and emergency paediatric policlinics of Erciyes University Gevher Nesibe Hospital between 01.06.1998 and 30.09.1998. A questionnaire including 54 questions was applied to the relatives of the children in the study group through face to face method. Other data was obtained from doctors and hospital records. The age of the child, educational level of the mother, monthly income and social insurance of the family, duration of diarrhoea, daily defecation number, vomiting and the result of stool culture were taken as independent variables, and the dehydration in the child was taken as a dependent variable. The effects of the independent variables on dehydration were analysed by logistic regression method. Results: Dehydration was diagnosed in 18.9 % of the study group. According to the results of univariable statistical methods, the effects of the age, daily defecation number, vomiting of the child, mother's educational level, social insurance and monthly income of the family on dehidratation were found statistically significant. But according to the results of logistic regression analysis, only the age, vomiting and daily defecation number of the child and the family income were found affecting the probability of dehydration significantly. Conclusion: The most significant factors affecting dehydratation probability in the children with diarrhoea were found the age, vomiting and daily defecation number of the child and the family income. The results of univariable and multivariable statistical methods were found different from each other.

Amaç: Bu çalisma, bazı sosyal, demografik ve tıbbi faktörlerin, ishalli çocuklarin dehidratasyona girme olasılığını ne ölçüde etkilediğini belirlemek amacıyla yapılmıştır. Gereç ve Yöntem: Araştırma 1998 yılı Haziran-Eylül ayları arasında Erciyes Üniversitesi Gevher Nesibe Hastanesi'nin çocuk polikliniği ve çocuk acil polikliniğine ishal nedeniyle getirilen 0-5 yaş grubundaki 164 çocuk üzerinde yapılmıştır. Araştırma kapsamına alınan çocuklari hastaneye getiren yakınlarına yüzyüze görüşme yöntemiyle 54 soru içeren bir anket formu araştırmacılar tarafından uygulanmış, diğer veriler ise hasta dosyaları incelenerek ve poliklinik hekimi ile görüşülerek elde edilmiştir. Çocugun yaşi, annenin ögrenim durumu, ailenin geliri ve sosyal güvencesi, ishalin süresi, günlük dışkılama sayısı, kusma durumu, dışkı kültüründe üreme durumu bağımsız değişkenler olarak, çocukta dehidratasyon olma durumu ise bağımlı değişken olarak alınmıştır. Bağımsız değişkenler kümesinin dehidratasyon durumuna etkisi logistik regresyon yöntemiyle analiz edilmiştir. Bulgular: Araştırma grubundaki çocuklarin %18.9'unda dehidratasyon tespit edilmiştir. Tek değişkenli istatistiksel analiz sonuçlarına göre; çocugun yaşi, dışkılama sıklığı, kusma durumu, annenin ögrenim durumu, ailenin sosyal güvence durumu ve aylık gelirinin, dehidratasyon olasılığını önemli ölçüde etkilediği bulunmuştur. Buna karşilık; logistik regresyon analizi sonucunda, sadece çocugun yaşi, dışkılama sıklığı ve kusma durumu ile aile gelirinin dehidratasyon gelişme olasılığını önemli ölçüde etkilediği belirlenmiştir. Sonuç: İshalli çocuklarda dehidratasyon gelişmesini etkileyen en önemli faktörler; çocugun yaşi, günlük dışkılama sayısı, kusma durumu ve ailenin ekonomik durumu olup, tek değişkenli istatistiksel analizlerle, logistik regresyon analizi sonuçları birbirinden farklı bulunmuştur.
