Abstract

Donohue's syndrome or leprechaunism is characterized by prenatal onset growth retardation, lacking subcutaneous fat and decreased muscle mass, insulin resistance and other phenotypic and hormonal changes. Metabolic characteristics of the disease include postprandial hyperglycemia, fasting hypoglycemia, insulin resistance, hyperinsulinemia, and failure to thrive. Most of the cases have severe insulin resistance with the mutation of the insulin receptor gene.The physical features most often associated with this condition including hypertrichosis, pachyderma, acanthosis nigricans, prominent genitalia, and elfin-like facial characteristics of prominent eyes, wide nostrils, thick lips, and large, low-set ears. Özet

Donohue sendromu prenatal dönemde baslayan büyüme geriliği, tipik yüz görünümü, cilt altı yağ dokusu ve kas dokusunda azalma ve insülin direnci ile karakterize otozomal resesif geçişli bir hastalıktır. Birçok fenotipik özellikler yanında değişik hormonal anormallikler de bildirilmiştir. Hastaliğin tipik metabolik bulguları postprandiyal hiperglisemi, preprandiyal hipoglisemi, insülin direnci, hiperinsulinemi ve büyüme geriliği olarak gözlenmektedir. Bu klinik duruma en sık eslik eden fizik bulgular akantozis nigrikans, klitoris hipertrofisi ve "elfin face" yüz görünümüdür. Bu yüz görünümü belirgin canlı bakan gözler, hipertelorizm, geniş ve antevert burun delikleri, kalın dudaklar, büyük, deforme ve düşük kulaklar şeklindedir.
