Pancreatic reserves in children who are carrying the risk factors of diabetes

Abstract

Different groups carrying the risk factors of diabetes, except autoimmune type I diabetes mellitus were studied in our clinic. These groups were; group I: the siblings of patients with type I diabetes mellitus (n:12), group II: patients with thalessemia major (n:41), group III : patients who developed high blood glucose and/or HbAlc levels during diabetic drug usage (group IIIa: corticosteroid and/or L-asparaginase usage (n:12), group 1Mb: growth hormone (GH) usage (n:8). Pancreatic reserves were evaluated by measurement of pre/postprandial C-peptide levels, stimulated C-peptide levels with glucagon and acute insuline response to intravenous glucose tolerance test (IVGTT), in 73 subjects. The results were compared with the healthy control group. There was no significant differences in the result of C peptide levels (pre/postprandial and glucagon stimulated) between group I and control group. But acute insuline responses to IVGTT were significantly lower in the group I in compared with the control group. In group II who had thalessemia major, 6 patients showed semptomatic (3 had permanent DM, 1 had temporary DM and 2 had stress hyperglisemia). 36 patients with thalessemia major were asymptomatic. Glucose intolerance in asymptomatic thalessemia major patients had significant increase in their pre and postprandial C-peptide levels, but glucagon stimulated C peptide levels were in normal value. Acute insulin response to IVGTT were significantly low in this group. The evaluation of pancreatic reserves in group IIIa showed that their postprandial C-peptide levels were high in compared with control group. The increase in C-peptide levels were assumed to be a compensatory responded of pancreatic beta cells in 137 GH deficient patients on GH therapy for 282 patients years and 2 of them developed DM where 6 had increased HbA1c levels (group 1Mb). The pancreatic reserve studies of these patients (group 1Mb) showed that there were no significant differences among C-peptide levels , but acute insulin response was significantly low in compared with the control group .With these results it is obvious that in children who are asymptomatic but carry the risk for DM, evaluation of pancreatic reserves in prediabetic period and preventive approach is very important.

Bilim dalımızda otoimmün primer Tip I Diabetes Mellitus (DM) olguları dışında diabetojenik risk faktörleri taşıyan; Grup l: Tip l DM'li çocukların kardeşleri (n:12), Grup II: thalessemi majör (n: 41), Grup III : diabetojenik ilaç kullanımı sırasında açık kan şekeri ve / veya HbAlc yüksekliği gelişen olgular, (Grup III a: steroid ve/yada L-asparaginaz kullanımı (n: 12), Grup 111 b: büyüme hormonu kullanımı (n:8)( olmak üzere toplam 73 olguda pankreas rezervi; açlık (ACP), tokluk (TCP), glukagonla uyarılmış (UCP) C peptid düzeyleri ve intravenöz glukoz uygulamasına (IVGTT) akut insulin yanıtının belirlenmesi ile denetlenmiştir. Sonuçlar sağlıklı kontrol olguları ile karşılaştırılmıştır. Tip l DM' li hastaların kardeşlerinden oluşan l. gruba pankreas rezerv denetimi yapıldığında açık, tokluk ve glukagonla uyarılmış C peptid yanıtlarında, kontrole göre istatistiksel olarak anlamı farklılık bulunmamış ise de intravenöz glukoz uygulamasına akut insulin yanıtlarında kontrole göre anlamı derecede düşük değerler elde edilmiştir. Grup II olarak verilen thalessemi major'lü 42 olgudan 3'ünde diabetik ketoasidozla prezente olan kalıcı diabet, 1'inde otoimmün hemolitik anemi eşliği nedeniyle steroid kullanımı sırasında gelişen geçici diabet ve 2 hastada da stres hiperglisemisi gelişmiştir. Diabet açısından asemptomatik olan 36 thalessemia majörlü olgunun pankreas rezerv denetimlerinde ise ACP ve TCP düzeylerinde kontrole göre belirgin artış saptanırken, glukagona C peptid yanıtında fark saptanmamıştır. IVGTT'de ise akut insulin yanıtları anlamı derecede düşük bulunmuştur. Grup IIIa' nın pankreas rezerv denetimlerinde TCP düzeyleri kontrole göre anlamı düzeyde yüksek saptanmıştır. C peptid düzeylerindeki artış stres altındaki pankreas beta hücrelerinin kompansasyon çabası olarak yorumlanmıştır. Boy kısalığı nedeniyle büyüme hormonu (BH) tedavisi alan 137 olgunun toplam 282 hasta yılı izleminde 2 hastada belirgin DM ve 6 hastada da HbA1c yükselmesi saptanmıştır (Grup III b). HbA1c yüksekliği saptanan bu hastaların pankreas rezerv denetimlerinde ise C Peptid düzeylerinde anlamı farklılık saptanmamış, fakat akut insulin yanıtları kontrole göre belirgin düşük bulunmuştur. Bu sonuçlar diabetojenik risk taşıyan çocuklarda henüz semptomatik olmadan, prediabet dönemde pankreas rezervinin belirlenmesi ve gerekli önlemlerin alınması bakımından önem kazanmaktadır.
