Effects of hyperinsulinemia on ADP and collagen induced platelet aggregation in hypertensive patients

Abstract

Essential hypertension is an important risk factor for coronary heart disease, peripheral vascular disease, stroke and congestive heart failure. In previous studies insulin resistance and, hyperinsulinemia has been determined in hypertensive patients. Platelets carry insulin receptors which are like other cells. Insulin effects those receptors by bounding for organising the platelets function by physiologically. It has been shown that aggregation of platelets can be decreased by giving insulin in healthy people. Unstable angina pectoris, acute myocardial infarction, sudden death, stroke, peripheral vascular disease are closely related with the changes of platelet aggretion for that reason it is important to understanding of effects of hyperinsulinemia in hypertensive patients on platelet aggregation. In this study; hyperinsulinemia has been created at 10 essential hypertensive and 10 healthy subjects by hyperinsulinemic eauglicemic clamp technique. Platelet aggregation which has been induced by ADP and collagen both before and after hyperinsulinemia has been studied. Result have been valued as maximum aggregation percentage was decreased from %69.50±4.09 to %51.74±5.56 (z=-2.8031, p=0.0051) by ADP and was decreased from %70.15±3.44 to %54.56±4.21 (z=-2.8031, p=0.0051 by kollagen. In fact maximum aggregation percentage of hypertensive patients for ADP and collagen was not statistically significant. From the other hand, maximum aggregation percentage before the hyperinsulinemia of hypertensive patients was statistically significant according to healthy subjects (for ADP Z=-3.6380 P=0.0003, for collagen z=-3.2261, p=0.0013). We claimed in this study that platelet aggregation is decreased by hyperinsulinemia in healthy subjects. Unfortunately insulin doesn't make antiaggregant effect in hypertensive patients, that station doesn't decrease platelet aggregation. That decreasing in anti-aggregation effect of insulin plays role mortality and morbidity of hypertension.

Hipertansiyon günümüzde en önemli morbidite ve mortalite nedenlerinden birisidir. Esansiyel hipertansiyon olgularında insülin rezistansı ve hiperinsülinemi gösterilmiştir. Trombositlerde insülin reseptörleri bulunmakta ve trombosit fonksiyonlarının düzenlenmesinde insülin önemli rol oynamaktadır. Sağlıklı kişilerde hiperinsülinemi trombosit agregasyonunu azaltmaktadır. Trombosit agregasyonundaki değişiklikler iskemik kalp hastalığı ve komplikasyonlarının oluşmasını direk etkilemektedir. Bu çalışmada Hipertansif hastalarda hiperinsilüneminin trombosit agregasyonuna etkilerini göstermeyi amaçladık. Çalışmaya 10 esansiyel hipertansiyon olgusu ve kontrol grubu olarak 10 sağlıklı olgu alındı. Hiperinsülinemik Öglisemik Klemp tekniği ile hiperinsülinemi oluşturuldu. Hiperinsülinemi öncesi ve sonrası ADP ve Kollajenle uyarılmış trombosit agregasyonu çalışıldı. Sonuçlar maksimum agregasyon yüzdesi olarak değerlendirildi. Kontrol grubunda hiperinsülinemi oluşturulduğunda: ADP ile maksimum agregasyon yüzdesinin %69.50 (4.09'dan %51.74±5.56'ya (z=2.8032, p=0.0051); kollajenle, %70.15 ±3.44'den % 54.56±4.21'e (z=2.8031, P=0.0051) düştüğü görüldü. Hiperinsülinemi öncesi hipertansif hasta grubunun maksimum agregasyon yüzdeleri sağlıklı gruba göre istatistiki olarak anlamlı derecede yüksek bulundu. (ADP için; %81.06±6.06, z=3.6380, p=0.0003, Kollajen için; % 82.74±7.65 z=3.2261, p=0.0013). Hipertansif hasta grubunda hiperinsülinemi sonrası değerler ADP ile %82±6.02, kollajenle %82±7.11 olarak bulundu ve bu bulgular, hiperinsülinemi öncesi değerlerle karşılaştırıldığında istatistiki anlamlılık saptanmadı. Hiperinsülineminin sağlıklı bireylerde trombositler üzerine düzenleyici etkilerinden birisi olan antiagregan etkisinin hipertansif hastalarda görülmemesi, hipertansiyonun mortalite ve morbiditesinin oluşmasında insülinin antiagregan etkisindeki azalmanın katkısı olduğunu göstermektedir.
