Long term follow-up of our patients with type I diabetes mellitus

Abstract

Type-I Diabetes Mellitus is a chronic disease with acute and chronic complications. In the follow-up most of the efforts are focused on to decrease the mortality and the morbidity. We planned this study to determine the long term evaluation of our patients. This study is a retrospective study including 31 cases who came to out- patient controls regularly at the last 10 years. Thirty one patients with Type I Diabetes Mellitus were evaluated by anthropometric measurements, insulin shemes, duration of remissions, remission C-peptide levels, HbA1c levels, lipid profiles, and late complications. The mean chronological age was 12.2±4.74 years (3.7-20.06), mean age at diagnosis was 7.77±4.0 years (1.3-16.5). The mean height SDS at admission was -0.10±0.98 and the target height SDS was -0.10±0.98. Their mean duration of the follow-up was 56.32±48 months and 71 % of the cases were having intensive insulin treatment. Of the cases 39% were prepubertal and 61 % were pubertal. Mean relative body mass index of our cases was 96.9±13.3 in whom 2 obesity cases were seen. Both of the cases were female, adolescent, and were receiving intensive insulin treatment. At admission fasting and postprandial mean C-peptide levels were respectively 0.95±0.93 ng/ml and 1.63±1.26 ng/ml, mean total cholesterol/HDL-cholesterol ratio was 3.79±1.33. Remission was observed in 14(%45) cases. Of these cases complete remission was observed in 7%, and-partial remission was observed in 93%. Duration of the remission was 11.46±6.61 (2-24) months. In remission fasting and postprandial C-peptide levels were respectively 1.65±1.41 ng/ml and 3.23±2.20ng/ml. All of the patients were in very good and fair metabolic control in the first, 4th and 5th years of the follow. At the second year 75%, and at the third year 73% of the patients were in very good and fair metabolic control. The mean total cholesterol/HDL-cholesterol ratios during the 5 years were respectively 3.4±0.95, 3.15±0.96, 3.82±1.06, 3.91±1.38, 3.49±0.67. Hospitilization due to hypoglycemia was 10 times, due and ketoacidosis was 11 times. In 3% retinopathy, in 19 % arthropathy, in 16% incipient nephropathy, and in 26% mild demyelinization neuropathy which could be demonstrated by ENMG were observed. Otoimmunity was observed in 6 (19%) patients (KLT in 4, gluten enteropathy in 2). Final height SDS was -0.32±1.17 in patients who reached their final height. There was no significant difference between the height SDS at admission and the final height SDS. Development of type I DM did not effect negatively the final height of our cases. Dyslipidemia known to be the reason for macrovascular diseases was not a problem in our 5 years follow-up. In our cases subclinic symptoms of late complications which were mostly incipient nephropathy and demyelinization in ENMG, were as in same ranges that was described in literature. Follow-up for late complications is also important in childhood and adolescent diabetes to catch the reversible phases.

Tip I DM akut ve kronik komplikasyonları olan kronik seyirli bir hastalıktır. İzlemde mortalite ve morbiditeyi azaltmaya yönelik çabalar ön planda tutulmaktadır. Bu nedenle hastalarımızın uzun süreli takipdeki durumunu ortaya koymak amaçlı bu çalışmayı planladık. Son 10 yıl içindeki düzenli kontrole gelen 31 olgu retrospektif olarak değerlendirildi. Toplam 31 Tip I DM'lu hasta antropemetrik ölçümleri, insulin cemaları, remisyon süreleri, remisyon C-peptid düzeyleri, HbA1c düzeyleri, lipid profilleri, ve geç komplikasyon taramaları açısından değerlendirildi. Bulgular: Olguların takvim yaş ortalamaları 12.2±4.74 (3.7-20.6), ortalama tanı yaşları 7.77±4.0 (1.3-16.5) desimal yıl, başvurularında ortalama boy SDS leri -0.10±0.98 ve hedef boy SDS'leri -0.56±0.99'dur. Ortalama izlem süreleri 56.32±48 ay olup %71'i yoğun insulin tedavisi kullanmaktadır. Hastaların %39'u prepubertal, %61'i pubertal evrededir. Ortalama ağırlık boy indeksleri 96.9±13.3 olan olgular arasında obezite ikisinde saptanmıştır. Her iki olgu da kız, adolesan ve yoğun insulin tedavisi altındadır. Başvurularında ortalama açlık ve tokluk C-peptid düzeyleri sırasıyla 0.95±0.93 ng/ml ve 1.63±1.26ng/ml, ortalama total kolesterol/H DL-kolesterol oranları 3.79±1.33'dür. Remisyona giren 14 (%45) olgunun %7'sinde komplet remisyon, %93'ünde parsiyel remisyon gözlenmiştir. Remisyon süreleri ortalama 11.46±6.61 ay (2-24), remisyonda ortalama açlık ve tokluk C-peptid düzeyleri sırasıyla 1.65±1.41 ng/ml ve 3.23±2.20ng/ml'dir. Hastaların tamamı birinci, dördüncü, ve beşinci yıllarında iyi ve orta kontrolde iken, 2. yılda % 75'i ,3. yılda % 73'ü iyi ve orta kontroldedir. Beş yıllık ortalama kolesterol, HDL kolesterol oranları sırasıyla 3.4±0.95, 3.15±0.96, 3.82±1.06, 3.91±1.38, 3.49±0.67 olarak bulunmuştur. Ağır hipoglisemi atağı ile 10, ketoasidoz tablosu ile 11 kez hastaneye yatış gözlenmiştir. Hastaların %3'ünde retinopati, %19'unda artropati, %16'sında insipient nefropati, ve %26'sında ENMG ile tesbit edilebilen hafif demyelinizasyon şeklinde nöropati gelişmiştir. Otoimmun hastalıklarla birliktelik 6 (%19) olguda saptanmıştır (4 KLT, 2 glüten enteropatisi). Final boya ulaşanların final boy SDS'leri -0.32±1.17 olarak bulunmuştur. Başvuru boy SDS'leri ile final boy SDS'leri arasında fark bulunamamıştır. Tip I DM gelişimi olgularımızın final boylarını olumsuz etkilememiştir. Makrovasküler hastalığa zemin hazırladığı bilinen dislipidemi 5 yıllık izlemde sorun olarak karşımıza çıkmamaktadır. Geç komplikasyonların subklinik belirtileri bizim olgularımızda da literatürde belirtilen oranlarda dikkati çekmekte olup en fazla insipient nefropati ve ENMG'de de hafif demyelinizasyon saptanmıştır. Çocuk ve adolesan diabetiklerinde de kronik geç komplikasyonlar açısından izlem geriye dönüşümlü dönemlerin yakalanması açısından önemlidir.
