Tip 2 Diabetes Mellitus'ta guatr oranı

Abstract
In this study 65 diabetic patients (46 female, 19 male; ages 55.42±11.38 years) followed by the endocrinology department of our hospital and 33 nondiabetic healthy subjects (22 female, 11 male;ages 50.42±13.95 years) were compared according to their thyroid hormone total triiodothyronin (tT3), total tetraiodothyronin (tT4), free triiodothyronin (fT3), free tetraiodothyronin (fT4), thyrothropin (TSH) and according to their having goiter or not determined by ultrasound. Besides diabetic patients were evaluated for metabolic regulation of diabetes, nephropathy, retinopathy and neuropathy. In the diabetic group 69.2% of the patients (36 female,9 male (45); 15 soliter, 37 multinodular, 1 diffuse and 1 operated goiter) had goitre while 42.4% (10 female, 4 male (14); 5 soliter, 9 multinodular goiter) of the control group had goiter. The difference between the goiter rates of the two groups was significant (p=0.01). Serum thyroid hormone (tT3, tT4, fT3, fT4, TSH) levels were in normal ranges in both of the groups and the difference was not significant (p>0.05). We found the goiter rate significantly increased the diabetics, but the thyroid hormone levels did not show any difference. As there is not much such study on DM type 2 patients we suggest further investigation with larger serials for more detailed results and conclusions.

Bu çalışmada hastanemiz endokrinoloji polikliniğinde takip edilen 65 diabet hastası (46 kadın, 19 erkek; yaş ortalaması: 55.42±11.38 yıl) 33 diabeti olmayan sağlıklı birey ile total triiodotironin (tT3), total tetraiodotironin (tT4), serbest triiodotironin (sT3), serbest tetraiodotironin (sT4), tirotropin (TSH), ve tiroid ultrasonografisi ile guatr varlığı açısından karşılaştırılmıştır. Diabetik hastalar ayrıca diabetin metabolik regulasyonu, nefropati, nöropati, retinopati yönünden de değerlendirilmiştir. Diabetik grupta bulunan hastalarda % 69.2 (36 kadın, 9 erkek (45); 15 soliter, 37 muitinoduler, 1 diffüz, 1 opere) oranında guatr saptanırken kontrol grubunda bu oran % 42.4 (10 kadın, 4 erkek (14); 5 soliter, 9 muitinoduler) olarak belirlenmiştir.Her iki grupta guatr oranları arasındaki fark istatistiksel olarak anlamlı bulundu (p=0.01). Serum tiroid hormon (tT3, tT4, sT3, sT4, TSH) seviyeleri her iki grupta da normal sınırlar içerisindeydi. Sonuç olarak bu çalışmada diabetik hastalarda guatr oranı sağlıklı kontrollere göre anlamlı olarak artmış bulunurken tiroid hormon değerlerinin farklı olmadığı saptanmıştır. Yapılan literatür taramasında tip2 diabetik hastaların guatr oranını değerlendiren çalışmaya rastlanmamıştır. Detaylı sonuç ve değerlendirmeler için daha geniş seriler ile çalışmaların artırılmasının uygun olacağı sonucuna varılmıştır.
