Silent myocardial ischemia in patients with type 2 diabetes mellitus: Which patients are under the risk?

Abstract
Coronary artery disease (CAD) is the major cause of mortality and morbidity in patients with type 2 diabetes mellitus. CAD is usually advanced when it is diagnosed and has an unfavorable prognosis. Late diagnosis of CAD could be explained by the presence of silent myocardial ischemia(SMI). SMI can be defined as, thepresence of myocardial ischemia without the chest pain. The purpose of this study was to define high risk, asymptomatic diabetic population which require screening SMI. Type 2 diabetic 82 patients (40 male, 42 female) were included to the study. Major cardiovascular risk factors and diabetic complications like retinopathy or neuropathy were recorded. Microalbuminuria and HbAIc level were also recorded. Exercise test was used as screening test for SMI on clinically asymptomatic patients with completely normal ECGs. If exercise test was succesfully completed, SMI was ruled out. If exercise test was interpreted as positive, coronary angiography was performed. Exercise test were positive in 16 of 82 patients. Coronary anjiography was performed 13 of 16 patients. Ni-ne patients (7 male, 2 women) had clinically significant coronary stenosis. There was a significant correlation between coronary stenosis and male sex, microalbuminuria level and family history of CAD. There was no correlation between coronary stenosis and age, duration of diabetes, lipid levels, HbAIc, body mass index and retinopathy. As a conclusion, microalbuminuric type 2 diabetic men with a positive family history of CAD was under high risk for SMI and must be followup carefully

Tip 2 diabetes mellitus'lu hastalarda mortalite ve morbiditenin en önemli nedeni koroner arter hastalığıdır (KAH), diabetik olgularda KAH tanı konulduğunda genellikle oldukça ilerlemiştir ve bu hastalarda prognoz kötüdür. Geç tanı konulmasının başlıca sebeplerinden biriside sessiz miyokard iskemisidir(SMİ). SMİ, miyokard iskemisinin göğüs ağrısı olmaksızın asemptomatik olarak görülmesi şeklinde tarif edilebilir. Çalışma asemptomatik diabetik bir popülasyonda SMİ için tarama gerektiren yüksek riskli bir grubu tanımlayabilmek amacıyla planlanmıştır. Çalışmaya 82 diabetik hasta (40 erkek, 42 kadın) dahil edildi. Kardiyovasküler risk faktörleri, retinopati ve nöropati gibi diabetik komplikasyonlar kaydedildi. Mikroalbüminüri ve HbAIc düzeyleri çalışıldı. Klinik olarak asemptomatik ve EKG leri tamamen normal hastalara, tarama testi olarak egzersiz testi uygulandı. Egzersiz testi negatif olan hastalarda SMİ dışlandı, pozitif bulunanlara ise doğrulama testi olarak koroner anjiografi uygulandı. Toplam 82 hastanın 16 tanesinde egzersiz testi pozitif saptandı. Bu 16 hastanın 13 tanesine koröner anjiografi uygulandı. Dokuz hastada (7 erkek, 2 kadın) klinik olarak anlamlı stenoz tespit edildi. Koroner stenoz ile, erkek cinsiyet, mikroalbüminüri ve KAH aile öyküsü arasında anlamlı bir ilişki mevcuttu. Ancak yaş, diabet süresi, lipit seviyeleri, vücut kitle indeksi, HbAIc ve retinopati arasında anlamlı bir ilişki saptanmadı. Sonuç olarak mikroalbüminürisi mevcut, ailesinde KAH öyküsü saptanan tip 2 diabetes mellitus'lu erkek hastalar SMİ açısından risk altındadır ve dikkatle izlenmeleri gerekir
