Relationship of the erythrocyte osmotic fragility and malondialdehide levels with complications in type 2 diabetes

Abstract
Objective: The erythrocyte membrane is susceptible to osmotic, oxidative and mechanical stress. Microvascular and macrovascular complications of type 2 diabetes mellitus can cause hypoxia leading to oxidant stress. Oxygen free radicals from hemoglobin occurs in erythrocytes, and erythrocytes becomes more fragile. The aim of this study is to investigate the relationship between erythrocyte osmotic fragility and complications; and to understand whether there is a correlation between malondialdehide which is an indirect marker of oxidant stress.Methods: A total of 46 type 2 diabetic subjects (21 with complications, 25 without complications; 25 female and 21 male) with mean age 57.8 ± 9.5 years were studied prospectively. The control subjects consisted of 25 patients (12 males, 13 females) with mean age 57±6.9 years. Fasting blood samples were collected into both plain tubes and tubes with K3 EDTA and analyzed in two hours for erythrocyte osmotic fragility (EOF) with method described by Merck Manuel. Malondialdehyde (MDA) concentrations in serum were detected in terms of TBARS (thiobarbituric acid reactive substances), spectrophotometrically by the method of Yoshioka ve Kawada . Amaç: Eritrositlerin karşılaştıkları osmotik, oksidatif ve mekanik stres doğrudan eritrosit membranını etkilemektedir. Tip 2 diabette vasküler komplikasyonlar hipoksik ortam oluşturarak eritrositlerde hemoglobin kaynaklı serbest radikaller oluşmasına yol açarlar ve eritrositlerin daha frajil hale gelmesine neden olabilirler. Bu çalışmada komplikasyon oluşmuş ve oluşmamış tip 2 diabet olgularında eritrosit osmotik frajilitesinin (EOF) incelenmesi ve bunun oksidan stresin indirek göstergesi olan malondialdehid (MDA) ile korelasyonunun araştırılması amaçlandı. Yöntem: Tip 2 diabet tanısı ile izlenen ve eşlik eden ağır hastalığı olmayan 21'i komplikasyonsuz, 25'i komplikasyonlu olmak üzere toplam 46 hasta (25 kadın, 21 erkek; yaş: 57.8±9.5) çalışmaya alındı.
