The risk factors affecting transient hypocalcemia in patients undergoing thyroidectomy

Abstract

Postoperative hypocalcemia is a common and most often transient event after extensive thyroid surgery. The causes of hypocalcemia are multifactorial. The purpose of this study was to evaluate the incidence and risk factors of transient hypocalcemia following thyroidectomy. A retrospective study was performed on 546 patients undergoing thyroidectomy. The rates of hypocalcemia were evaluated based on age, gender, presence of associated carcinoma, extent of resection, primary or secondary surgery, hyperthyroidism, whether parathyroid autotransplantation was performed. A linear regression model was used to determine independent risk factors. The overall incidence of transient and permanenet hypocalcemia was 9.9% and 0.18%, respectively. On linear regression analysis, the extent of surgery and hyperthyroidism constituted independent risk factors for transient hypocalcemia. The incidence of transient hypocalcemia was 16.67% in patients with total thyroidectomy, 8% in patients with subtotal thyroidectomy, 3.45% in patients with unilateral lobectomy, 20.45% in patients with hyperthyroidism, and 7.86%in patients without hyperthyroidism. The extent of surgery and hyperthyroidism had a significant effect on the incidence of transient hypocalcemia following thyroid surgery. High-risk procedures, such as total thyroidectomy and patients with hyperthyroidism should be followed more carefully for hypocalcemia during postoperative period.

Postoperatif hipokalsemi sıktır ve geniş tiroid cerrahisinden sonra bile sıklıkla geçicidir. Hipokalsemi”nin sebepleri multifaktöriyeldir. Bu çalışmada tiroidektomiyi takiben geçici hipokalseminin insidansı ve risk faktörlerini değerlendirmeyi amaçladık. Tiroidektomi uygulanan 546 hastada retrospektif çalışma uygulandı. Yaş, cinsiyet, tiroid kanseri varlığı, tiroidektominin genişliği, tiroidektominin primer veya sekonder olması, hipertiroidizm varlığı, paratiroid ototransplantasyonu yapılıp yapılmamasına göre hipokalsemi oranları değerlendirildi. Istatistiksel değerlendirmede bağımsız risk faktörlerini belirlemek için lineer regresyon analizi kullanıldı. Toplam geçici ve kalıcı hipokalsemi insidansları sırası ile %9.9 ve %0.18'di. Lineer regresyon analizinde geçici hipokalsemi için tiroidektominin genişliği ve hipertiroidizm varlığı bağımsız risk faktörleri olarak belirlendi. Geçici hipokalsemi insidansı, total tiroidektomide %16.67, subtotal tiroidektomide %8, tek taraflı lobektomide %3.45, hipertiroidizmli hastalarda %20.45, ötiroid hastalarda ise %7.86 idi. Tiroidektomiyi takiben cerrahinin genişliği ve hipertiroidizm varlığı geçici hipokalsemi insidansı üzerine anlamlı etkiye sahiptir. Belirlenen bu yüksek riskli durumlarda hastalar postoperatif dönemde hipokalsemi için daha dikkatli takip edilmelidirler.
