Gingival crevicular fluid and proteomics: could they be become diagnostic tools for autoimmune thyroiditis and thyroid carcinoma?

Abstract
Several clinical and laboratory researches are ongoing to clarify the unknowns of autoimmune thyroiditis and thyroid cancer. New methods in molecular biology such as genomics and proteomics technology have been introduced and numerous promising data are now available for clarification of the mechanisms of cancer development and for early diagnosis of cancer. However, tissue and blood sampling is essential for these studies. The use of large bore needle aspiration to obtain thyroid tissue has also been reported. However, the potential complications of such techniques forces the investigators to develop less invasive sampling methods. Gingical crevicular fluid is an ultrafiltrate of blood. It is possible to obtain gingival crevicular fluid with noninvasive techniques. Several studies have shown that numerous biomarkers can be obtained from gingival crevicular fluid and that these can be measurable. Although currently there is no specific data on the use of gingival crevicular fluid for studies on autoimmune thyroiditis and thyroid cancer, the emerging results of gingival crevicular fluid studies suggest that it has a potential to be a new investigation area in thyroid diseases.
Özet
Otoimmün tiroiditler ve tiroid kanserlerinin bilinmeyenlerini aydınlatmak için pek çok klinik ve laboratuvar çalışması devam etmektedir. Moleküler biyolojideki genomikler ve proteomikler ile ilgili teknoloji gibi güncel gelişmeler kullanıma girmiştir ve kanser gelişim mekanizmalarını açıklamak ve kanserin erken tanısında kullanıma ilişkin umut verici sonuçlar bildirilmektedir. Yine de bu çalışmaların çoğu, doku ve kan örneklemesi gerektirmektedir. Tiroid dokusu örneklemesi için dekalın iğne aspirasyonu ile yapılan çalışmalar bildirilmiştir. Fakat bu teknikler ile birlikte olan potansiyel komplikasyonların varlığı araştırmacıları daha az girişimsel olan örnekleme yöntemleri geliştirmeye zorlamaktadır. Dişeti oluğu sıvısı (DOS) bir kan ultrafiltratıdır. DOS’un girişimsel olmayan yöntemlerle elde edilmesi mümkündür. Ayrıca bu sıvıda sayısız biyo-işaretleyicinin elde edilebildiği ve ölçülebildiği bildirilmiştir. Her ne kadar bugün için otoimmün tiroiditler ve tiroid kanserleri için spesifik bir veri yoksa da DOS üzerinde yapılan çalışmaların ilk sonuçları bu materyelin tiroid bezi ile ilgili yeni bir araştırma alanı olma potansiyeline sahip olduğunu düşündürmektedir.
