Complemantary role I-131 SPECT in substernal goiters

Abstract
In this study the effectiveness of single photon emission computed tomography (SPECT) in detection of substernal goiters has been investigated. The study group was consisted of 16 female and 6 male patients with a mean age of 54.8 and age range of 26-75. We aimed to correlate the results of functional imaging with the results of anatomical imaging methods and intra-operative topographic findings in patients who underwent surgery. Computed tomography (CT) and magnetic resonance imaging (MRI) were the selected anotomic imaging methods and they were accepted as gold standards together with surgical findings. SPECT was found to be beneficial in assessing the degree of extension and functional activity of retrosternal tissue. However, it has the disadvantage of higher dose administration compared to planar scintigraphy which is hardly acceptable regarding radiation dosimetry. For that reason it has been concluded that SPECT can only be applied in selected cases.
Özet
Substernal guatrlarda tek foton emisyon tomografisinin (Single Photon Emission Computed Tomography) (SPECT) araştırıldığı çalışmada İstanbul Üniversitesi İstanbul Tıp Fakültesi Nükleer Tıp Anabilim Dalı ve Endokrinoloji Bilim Dalı'na başvuran 16 kadın, 6 erkek toplam 22 hasta incelendi. Olguların yaş ortalaması 54.8, yaş aralığı 26-75 idi. Burada amaçlanan sintigrafi sonuçları ile altın standart olarak kabul edilen ve bütün hastalara uygulanan bilgisayarlı tomografi (BT) veya magnetik rezonans görüntüleme (MRI) bulgularının, opere edilen hastalarda intraoperatif topografik bulguların korrelasyonunun araştırılmasıydı. Elde edilen sonuçlara göre SPECT retrosternal dokunun uzanım yönünün belirlenmesinde ve fonksiyonel aktivitesinin belirlenmesinde faydalı bulunmuştur. Ancak, SPECT'in radyasyon dozimetresinin kabul edilebilir sınırlarda olmayışı, "spatial" rezolüsyonda kayıp ve kontrast rezolüsyona katkı sağlamama gibi dezavantajlarından dolayı seçilmiş olgularda yarar sağlayabileceği sonucuna varılmıştır.
