Midterm results of Salter and Pemberton pelvic osteotomies for developmental dysplasia of the hip

Abstract

Objectives: The aim of this study was to compare the clinical and radiographic results of Salter and Pemberton pelvic osteotomies in developmental dysplasia of the hip (DDH). Patients and methods: Thirty-three patients (43 hips) with DDH were retrospectively evaluated. Twenty-three patients (29 hips) underwent Salter (SPO), 10 patients (14 hips) underwent Pemberton (PPO) osteotomies. The mean age was 40.5 months (18 to 130 months) in the SPO group, and 27 months (18 to 52 months) in the PPO group. One-staged surgery was performed in all the patients. Following open reduction, pelvic and/or femoral osteotomies were performed. In patients with a thin iliac wing and adequate acetabular width, PPO was preferred, otherwise SPO was the choice. The acetabular index (AI) and center-edge (CE) angles were measured on pre- and postoperative radiographs. Radiographic and clinical results were assessed according to the Severin and modified McKay criteria, respectively. The mean follow-up was 38 months (12 to 75 months) in the SPO group, and 33 months (12 to 64 months) in the PPO group. Results: The mean postoperative AI and CE angles significantly improved in both groups (p0.05). Conclusion: Both osteotomy methods were found to be safe and successful for the treatment of DDH with anterolateral acetabular deficiency.

Amaç: Bu çalışmada, gelişimsel kalça displazisi (GKD) tedavisinde Salter (SPO) ve Pemberton (PPO) pelvik osteotomilerinin klinik ve radyografik sonuçları karşılaştırıldı. Hastalar ve yöntemler: Çalışmada, GKD’li 33 hastanın 43 kalçası geriye dönük olarak değerlendirildi.Yirmi üç hastaya (29 kalça) SPO, 10 hastaya (14 kalça) PPO uygulandı. Ortalama yaş SPO grubunda 40.5 ay (dağılım 18-130 ay), PPO grubunda 27 ay (dağılım 18-52 ay) idi. Tüm hastalarda tek aşamalı cerrahi uygulandı. Öncelikle açık redüksiyon denendi, stabil redüksiyon sağlanamayan hastalara aynı seansta femoral ve/veya pelvik osteotomi yapıldı. İliyak kanat kalınlığı ince ve asetabulum genişliği uygun ise PPO, yoksa SPO yapıldı. Hastaların ameliyat öncesi ve sonrasında pelvis grafilerinden asetabuler indeks (Aİ) ve merkez-kenar açısı (MKA) ölçüldü. Radyografik değerlendirme Severin ölçütlerine göre, klinik değerlendirme modifiye McKay ölçütlerine göre yapıldı. Ortalama takip süresi SPO grubunda 38 ay (dağılım 12-75 ay), PPO grubunda 33 ay (12-64 ay) idi. Bulgular: Salter ve Pemberton osteotomi gruplarında Aİ’deki düzelme sırasıyla 21.9° (%54.6) ve 21.4° (%58), MKA’daki düzelme 28.7° ve 27.9° bulundu. İki grup içinde Aİ ve MKA’daki düzelme tedavi öncesine göre anlamlı idi (p0.05). Sonuç: Her iki pelvik osteotomi yöntemi de, anterolateral örtünme sorunu olan asetabuler displazilerin tedavisinde güvenli ve başarılı bulundu.
