Access to Health Care Dimension in Health Inequalities: Review of OECD Countries

Abstract

Inequalities in health are one of the most important issues of international health agenda nowadays. According to World Health Organisation, there are wide disparities in the health status of different social groups in all countries in the World and the gap among countries and within countries are widening. There are two main reasons of health inequalities, one of which is socioeconomic and political determinants, and the other of which is insufficiencies in health system. In this study health inequalities are examined through health system insufficiencies. In this respect “access to health care” which is one of the intermediate goals of health systems and very important criteria for assessment of health inequalities will be examined. Before assessing access to health care, health status in OECD countries will be reviewed by life expectancy at birth and infant mortality rates. The study will be focused on access to health care in OECD countries and use OECD criteria. These criteria are: population coverage for health care, unmet needs for health care due to cost, out-of-pocket medical expenditure, geographic distribution of doctors, and waiting times for elective surgery. At the end of the study it is concluded that there are significant health inequalities and barriers to access to health care among OECD countries, and some suggestions to abolish these situations which are avoidable and unacceptable.

Özet

Sağlıktaki eşitsizlikler son yıllarda uluslararası sağlık gündeminin en önemli konularıarasındadır. Dünya Sağlık Örgütü verileri sağlıkta eşitsizliklerin tüm ülkeler için önemli birsorun olduğunu, yıllar itibariyle yalnızca ülkeler arasındaki uçurumun değil ülkelerin kendiiçlerindeki eşitsizliklerin de derinleştiğini göstermektedir. Sağlıkta eşitsizlikler genel olarak birisağlığın sosyoekonomik ve politik belirleyicileri, diğeri sağlık sistemi yetersizlikleri olmak üzereiki neden ile ilişkilendirilmektedir. Bu çalışmada sağlıkta eşitsizliklerin izi sağlık sistemindekiyetersizlikler üzerinden sürülmektedir. Bu kapsamda sağlık sisteminin ara hedeflerinden birinioluşturan ve sağlıkta eşitsizliklerde önemli bir rol oynayan “sağlık hizmetlerine erişim” konusuodak noktayı oluşturmaktadır. Sağlıkta erişim konusu ele alınmadan önce OECD ülkelerindesağlık durumunun genel bir görünümünü ortaya koymak için doğumda beklenen yaşam süresive bebek ölüm hızları ele alınmaktadır. Çalışma sağlık hizmetlerine erişimi OECD ülkeleriözelinde ve OECD ölçütleri ile incelemektedir. Bu ölçütler: sağlık güvencesi kapsamı, maliyetnedeniyle karşılanamayan sağlık ihtiyaçları, cepten yapılan sağlık harcamaları, hekimlerincoğrafik dağılımı ve acil olmayan (elektif) cerrahi için bekleme süreleridir. Çalışma sonundaOECD ülkelerinde sağlık hizmetlerine erişimde önemli engeller bulunduğu ve eşitsizlikleryaşandığı ortaya konulmakta, önlenebilir ve kabul edilemez nitelikte olan bu durumun ortadankaldırılması için önerilerde bulunulmaktadır.
