Evaluation of the cases with meconium aspiration syndrome: Clinical spectrum and our treatment modalities

Abstract
Despite the effective intrapartum and postnatal aspiration techniques, meconium aspiration syndrome (MAS) continues to be an important problem in newborns. In this study the data of 26 infants with MAS followed at our Neonatal Intensive Care Unit (NICU) during a period of 5 years and the incidence of meconium stained amniotic fluid (MSAF) in the babies delivered in Obstetrics Ward were analysed. The frequencies of respiratory distress, pneumonitis, pneumothorax and pulmoner hypertension were 84.6%, 61.5%, 11.5%, and 11.5%, respectively. Severe MAS necescitating mechanichal ventilation was found as 23.1%. All cases recovered including the severe ones to whom surfactant replacement and high frequency ventilation were performed (19.2%). Prenatal diagnosis and preventive measures seem to be much more critical than complicated treatment modalities for the solution of this problem.

Mekonyum aspirasyon sendromu (MAS), doğum sırası ve sonrasında uygulanan etkin aspirasyon yöntemlerine karşın yeni doğanlarda önemli bir sorun olmaya devam etmektedir. Gelişmiş ülkelerde bile tüm perinatal ölümlerin %2'sinden sorumludur. Bu çalışma ile Yenidoğan Yoğun Bakım Ünitesi (YYBÜ)'sinde 5yılda izlenen MAS'lu 26 bebeğin verileri ve Obstetri Kliniği'nde mekonyumla boyalı amniyon sıvısı (MBAS) insidansı incelenmiştir (%4.5). MAS'lu yenidoğanların %84.6'sında solunum sıkıntısı, %61.5'unda pnömonitis, %11.5'inde ise pnömotoraks ve pulmoner hipertansiyon gelişmiştir. Mekanik ventilasyon gerektiren ağır MAS sıklığı %23.1'dir. Surfaktan replasmanı ve yüksek frekanslı ventilasyon uygulaması yapılan bebekler (%19.2) de dahil olmak üzere tüm olgular düzelme ile sonlanmıştır. Bu sorunun prenatal tanı konarak önlenmesi komplike tedavi yöntemlerinden daha akılcı görülmektedir.
