The importance of iron therapy in children with breath holding spells

Abstract
Breath-holding spells are a common problem in early childhood. In recent studies, it is concluded that in some patients iron-deficiency anemia may be a factor contributing both to psychomotor development and breath-holding spells. We evaluated 50 children who had breath-holding spells hematologicially age of patients was between 6 months and 4 years. Patients who were diagnosed to have iron-deficiency anemia were given iron treatment for 3 months at a dose of 6 mg/kg/24h orally. Every month the decrease in the frequency of spells were noted and after 3 months we reevaluated the patients for remission after three months, children treated with iron, showed significant reduction in the frequency of spells. In the anemic group, complete and partial response to iron therapy was 85.3% and in the non-anemic group it was. 37.5%. The treated group showed a positive correlation between low hemoglobin levels and clinical response. For obstructive pathologies, otorhinolaryngology consultation was performed. 7 patients had adenoid hypertrophy, one patient had a thyroglassal duct cyst. Children with breath-holding spells have an underlying autonomic nervous system dysregulation. This dysregulation acts together with emotional factors and iron-deficiency anemia increases tendency toward breath-holding spells. We conclude that in the patients with anemia, iron treatment may reduce the frequency and severity of breath-holding spells.

Katılma nöbetleri, erken çocukluk çağında sık rastlanan bir problemdir. Son zamanlarda demir eksikliği anemisinin psikomotor gelişime etkisi ve katılma nöbetlerine eğilimi arttırması ile ilgili çalışmalar önem kazanmıştır. 50 katılma nöbetli çocuğun hematolojik değerlendirmesi yapıldı. Yaşları 6 ay-4 yaş arasında değişmekteydi. Demir eksikliği anemisi saptanan olguya, 3 ay süre ile 6 mg/kg/gün oral demir tedavisi verildi. Her ay nöbet sıklığındaki azalma tespit edilerek 3. ay sonrasında remisyon değerlendirildi. Üç aylık demir tedavisi sonrasında, anemili olguların aylık nöbet atağında azalma belirgin olarak bulundu. Tam ve kısmi yanıt anemili grupta %85.3, anemisi olmayan grupta ise %37.5 saptandı. Anemi grubunun, demir tedavisine yanıtında, hemoglobin düzeylerindeki düşüklük ile klinik yanıt arasında pozitif korelasyon bulundu. Obstrüktif patolojiler açısından KBB konsültasyonu yapıldı. 7 vakada adenoid hipertrofisi, 1 vakada tiroglossal duktus kisti saptandı. Katılma nöbetlerinde otonom disregülasyonla emosyonel faktörler etkileşmekte olup demir eksikliği bu yönelimi arttırmaktadır. Bu çalışma sonunda, katılma nöbetlerinin şiddeti ve sıklığının azaltılmasında anemili olgularda demir tedavisinin çok önemli olduğu ortaya konmuştur.
