Iron deficiency anemia in diabetes mellitus type 1 and the effect of anemia to HBA1c

Abstract
Glycemic control in Diabetes Mellitus in the last 2-3 months is referred by glycosylated hemoglobin A1c (HbA1c) however, last 2-3 weeks' glycemia control is referred by fructosamine. In this prospective study, patients following up with Type 1 Diabetes Mellitus diagnosis were appreciated according to the effect of iron deficiency anemia, to the HbA1c and fructosamine levels. The correlation among hemoglobin, HbA1c and fructosamine levels in formated two groups with iron deficiency anemia and without, it was statistically insignificant. Likely to it, relationship between two groups according to HbA1c levels was also insignificant. The patient with iron deficiency anemia orally administrated 5 mg/kg/day ferrose iron and at the end of the third months there was the increase of the value of hemoglobin from 9.5 gr/dl to the 11.5 gr/dl. In the same group the mean value of the HbA1c performed 15.5% decrease which was statistically important also corellated with steady state of the fructosamine values. In conclusion, in Type 1 Diabetes Mellitus patient with iron deficiency anemia the values of HbA1c were not so reliable therefore, fructosamine might be referred glycemic status of patient more sensitively during anemia then glycosylated hemoglobin.
Özet
Diyabetes Mellitus (DM)'da son 2-3 aylık glisemik kontrolü hemoglobin A1c (HbA1c), son 2-3 haftalık kontrolü ise früktozamin yansıtır. Tip 1 DM tanısı ile izlediğimiz olgularda prospektif bir çalışmayla HbA1c ve früktozamin değerlerininin demir eksikliği anemisinden etkilenip etkilenmediğini araştırdık. Demir eksikliği anemisi olan ve olmayan gruplardaki olguların ortalama hemoglobin değerleri ile HbA1c ve früktozamin değerleri arasında anlamlı bir korelasyon saptanmadı. Yine her iki grubun HbAlc'Ieri arasında istatistiki yönden anlamlı bir fark bulunmadı. Demir eksikliği anemisi olan gruba 3 ay süre ile 5 mg/kg dozunda oral ferroz demir uygulandı ve bu grubun ortalama hemoglobin değerleri 9.5 gr/dL'den 11.5 gr/dL'ye yükseldi. Aynı grubun ortalama HbA1c değerinde ise %15.5'lik istatistiki yönden anlamlı bir düşüş saptanırken ortalama früktozamin değerinde istatistiki yönden anlamlı bir değişiklik saptanmadı. Sonuç olarak demir eksikliği anemisi bulunan DM'li olgularda HbAlc'nin çok da güvenilir olmadığını; böyle olgularda özellikle anemi düzeltilinceye kadar glisemik kontrol için früktozamin kullanımının uygun olacağını düşünmekteyiz.
