Comparision of rectal diazepam and oral phenobarbital prophylaxis in the prevention of febrile seizures

Abstract
Phenobarbital that has been widely used in the prevention of febrile seizures is currently not favorite since its side effects and lack of the efficacy. In this study, we aimed to compare the rectal diazepam prophylaxis with the phenobarbital prophylaxis in the prevention of febrile seizures. A total of 81 children with febrile convulsions were randomized to either intermittent rectal prophylaxis with diazepam (0.5 mg Per kilograms of body weight) or oral phenobarbital prophylaxis (5 mg Per kilograms of body weight). The mean age of the whole group was 4.3 years, 52% of the cases were boys and 48% of them were girls. During a mean follow-up of 24.1 months, only 18.5% of our patients showed recurrence of febrile seizures. In the diazepam group, recurrence of seizures was 13.2%, while it was 23.2% in the phenobarbital group (p = 0.24). Gender, age and family history associated with epilepsy didn't seem to affect the recurrence of seizures (p = 0.91 by Levene's test for equality of variances). This study provide an additional support that intermittent rectal diazepam prophylaxis is an effective method in reducing the risk of febrile seizures.

Febril konvülsiyonların önlenmesinde yaygın olarak kullanılmış olan fenobarbital, yan etkileri nedeniyle daha az tercih edilmektedir. Bu çalışmada, febril konvülsiyonları önlenmede, rektal diazepam ve oral fenobarbital uygulamalarının etkinliklerinin karşılaştırılması amaçlanmıştır. Febril konvülsiyonlu toplam 81 çocuk, intermitant rektal diazepam (0.5 mg/kg) ve oral fenobarbital (5 mg/kg) tedavi gruplarına random olarak seçilmiştir. Tüm grup incelendiğinde yaş ortalaması 4.3 yıl olarak saptanmıştır. Olguların %52'si erkek, %48'i kız olarak belirlenmiştir. Ortalama izlem süresi 24.1 ay olup %18.5 hastada febril konvülsiyonlann tekrarı izlenmiştir. Bu oran diazepam grubunda %13.2 iken, fenobarbital grubunda %23.2 olarak saptanmıştır(p = 0.24;. Tüm olgularda, febril konvülsiyon tekrarı için cinsiyet, aile öyküsü ve 2 yaşın altında febril konvülsiyon geçirmenin anlamlı bir risk faktörü olmadıkları görülmüştür (p = 0.91). Bu çalışmada, intermitant diazepam proflaksisinin, febril konvülsiyon tekrarını önlemede etkin bir yöntem olduğu birkez daha gösterilmiştir.
