Role of leukotriene antagonists in asthma therapy

Abstract
Anti-leukotriene drugs, are a new model of pharmacotherapy in the treatment of chronic asthma. In the present study, it was aimed to estimate the superiority of combining a leukotriene antagonist with a corticosteroid over the use of a corticosteroid alone, and to evaluate whether it has a reducing effect on the corticosteroid dosage and the clinical benefits of combination therapy. A total of 50 pediatric patients (13 girls and 37 boys) aged between 6-14 years with mild or moderate persistent asthma followed in the outpatient allergy clinic were enrolled into the study. They have been using low (100-200 mcg) or moderate (200-400 mcg) doses of inhaled budesonide at least for six months The patients were randomized into two groups. The first group (n=32) was given 5 mg/day montelukast sodium and inhaled budesonide for 12 weeks, and the second group (n=18) was given only inhaled budesonide for the same period. Both groups were permitted to use ß2-agonist as needed. PEF values of Group 1 were significantly elevated as compared to Group 2 at the end of the 8th week (p=0.0075). Beta-2 agonist use was significantly decreased in Group 1 at the end of the 4th week as compared to Group 2 (p=0.008). Significant improvement has been achieved over control parameters of asthma by adding leukotriene receptor antagonists to the steroid therapy.

Lökotrien reseptör antagonistleri kronik astım tedavisinde yeni bir farmakoterapi biçimi olup, düşük ya da orta düzeyde inhale steroid dozlarıyla tedaviye rağmen semptomları devam eden hastalarda tedaviye eklenmesi önerilen ajanlardır. Çalışmamızda lökotrien reseptör antagonistlerinin inhale steroidlerle kombinasyonunun tek başına inhale steroid kullanımı ile karşılaştırılması ve kombinasyon tedavisinin klinik yararlarının araştırılması amaçlanmıştır. Allerji-Solunum Polikliniği tarafından hafif ya da orta persistan allerjik astım nedeniyle izlenen, en az 6 aydır düşük (100-200 mcg) veya orta (200-400 mcg) dozda inhale steroid (budesonide) kullanan, 13'ü kız (%26), 37'si erkek (%74) toplam 50 hasta çalışmaya alınmıştır. Grup l'deki 32 hastaya 8 hafta süre ile 5 mg/gün montelukast+inhale budesonide, Grup ll'deki 18 hastaya aynı sürede tek başına inhale budesonide verilmiştir. Hastaların atak sayıları, ek ß-2 agonist ihtiyaçları ve günlük PEF ölçümleri izlenmiştir. Çalışmanın sonunda Grup II ile karşılaştırıldığında Grup l'deki hastaların ortalama PEF değerlerinde anlamlı artma (p=0.0075), Beta-2 agonist kullanımlarında anlamlı azalma (p=0.008) görülmüş, ancak atak sayılarında farklılık saptanmamıştır. Sonuç olarak lökotrien reseptör antagonistlerinin inhale steroidlere eklenmesiyle astım kontrol parametrelerinde düzelme sağlanabileceği düşünülmüştür.
