The Relationship Between Disc Dislocation and Anatomical Factors in the Temporomandibular Joint

Abstract

Purpose: To evaluate the correlation between disc dislocation (DD) and the anatomical/structural factors includingdisc thickness, eminence type and height, intercondylar angle and distance in the temporomandibular joint (TMJ). Materials and Methods: A total of 108 joints (54 cases) with TMJ magnetic resonance imaging (MRI) and computed tomography (CT) between 2013 and 2017 were retrospectively examined.DD was evaluated on MRI. The thicknesses of anterior and posterior horns of the disc were measured on MRI; eminence type and height on the sagittal plane, intercondylar angle and distance on the axial plane were measured on CT. DD in closed mouth were classified as either with or with out reduction in open mouth examination. Eminence types were determined as follows; 1 box, 2 sigmoid, 3 flattened, 4 deformed. ANOVA test and Spearman correlation analysis were used for statistical analysis. Results: There was no significant correlation between DD with/without reduction and anterior horn thickness, eminence height, inter condylar angle, inter condylar distance (p=0,12, 0,782, 0,096, 0,167, respectively). Posterior horn of the disc was thinner in DD without reduction (p=0.029).There was a significant increase in DD without reduction while eminence type was changing from type 1 to 4 (p=0.037). Conclusion: Anatomical/structural differences are related with DD in the temporomandibular joint. Thinning of posterior horn of the disc, and flat-deformed type of eminence are correlated with the disc dislocation without reduction.

Özet

Amaç: Temporomandibular eklemde (TME); disk kalınlığı, Eminensia tipi ve yüksekliği, interkondiler açı ve mesafeyi içeren anatomik/yapısal faktörlerin disk dislokasyonu ile ilişkisinin değerlendirilmesidir. Gereç ve Yöntem: 2013-2017 yılları arasında TME manyetik rezonans görüntüleme (MRG) ve bilgisayarlı tomografisi (BT) olan 108 eklem (54 olgu)retrospektif olarak incelendi. MRG’den disk dislokasyonu olup olmadığına karar verildi. MRG’de diskin ön ve arka boynuzunun kalınlığı; BT’de sagital düzlemde eminensia tipi ve yüksekliği, aksiyal düzlemde interkondiler açı ve mesafe ölçüldü. Ağız kapalı incelemede disk dislokasyonu izlenen eklemler, ağız açık incelemeye göre redüksiyonlu veya redüksiyonsuz olarak sınıflandı. Eminensia tipi; 1 kutu, 2 sigmoid, 3 yassı ve 4 düzensiz olarak belirlendi. İstatistiksel incelemede ANOVA testi ve Spearman korelasyon analizi kullanıldı. Bulgular: Redüksiyonlu/redüksiyonsuz disk dislokasyonuyla ön boynuz kalınlığı, eminensia yüksekliği, interkondiler açı ve mesafe arasında anlamlı ilişki görülmedi (sırasıyla, p=0,12, 0,782, 0,096, 0,176). Redüksiyonsuz dislokasyona doğru ilerledikçe disk arka boynuzun inceldiği görüldü (p=0,029). Eminensia tip 1’den 4’e gidildikçe redüksiyonsuz dislokasyonda anlamlı artış izlendi (p=0,037). Sonuç:Temporomandibular eklemde yapısal/ anatomik farklılıklar disk dislokasyonunu etkiler. Disk arka boynuzunda incelme, yassı ve düzensiz Eminensia redüksiyonsuz disk dislokasyonuyla ilişkidir.
