VERTICAL TRANSMISSION AND NEONATAL–PERINATAL PERSPECTIVES OF COVID-19 INFECTION: SINGLE CENTER EXPERIENCE

Abstract

Objective: The effects of the prelabor detected maternal novel coronavirus (SARS-CoV-2) infection and the disease it causes (COVID-19), on the newborn babies are not clear yet. This case series is sharing the clinical experience of the Dokuz Eylul Unversity Hospital with COVID-19 associated pregnants and newborns from the date pandemic set in Turkey to date. Materials and Methods: 13 neonates born to 12 pregnant women who admitted to Dokuz Eylul University Hospital due to labor and detected SARSCoV- 2 in nasopharyngeal swab sample based on RT-PCR between March 2020 and February 2021 were evaluated. Results: No clinical signs of the COVID-19 were observed in newborns during hospitalization. Samples from cord blood, breast milk, amniotic fluid and nasopharyngeal swabs of neonates resulted negative for SARS-CoV-2. Maternal vascular malperfusion features were detected in 60% (6/10) of examined placental pathology. Conclusion: Current results are compliant with the previously published studies suggesting that COVID-19 infection in the third trimester may not be associated with vertical transmission. It indicates the importance of postnatal contact precautions and protective equipment in reducing intrapartum and postpartum transmission.

Özet

Amaç: Perinatal dönemde gebelerde saptanan yeni korona virüs (SARS-CoV- 2) ve neden olduğu hastalığın (COVID-19) yenidoğan bebeğe etkileri henüz kesinlik kazanmamıştır. Bu olgu serisi, ülkemizde pandeminin başlangıcından bugüne dek, Dokuz Eylül Üniversitesi Hastanesinde, COVID-19 ile ilişkili gebeler ve bebeklerindeki klinik deneyimimizi paylaşmaktadır. Gereç ve Yöntem: Mart 2020 – Şubat 2021 tarihleri arasında planlı doğum veya spontan başlayan doğum eylemi nedeniyle Dokuz Eylül Üniversitesi Hastanesine başvuran, nazofarengeal sürüntü örneklerinde SARS-CoV-2 pozitifliği saptanan, birinde ikiz gebelik olan, toplam 12 gebe ve 13 bebek değerlendirilmiştir. Bulgular: Yenidoğan yoğun bakımda takip edilen bu bebeklerin takipleri boyunca, COVID-19 ile ilişkili bir semptomları gelişmemiş; kord kanı, amniyon sıvısı, anne sütü ve nazofarengeal sürüntü örneklerinde viral RNA saptanmamıştır. Değerlendirilen plasenta patolojilerinin altısında (%60) maternal vasküler malperfüzyon saptanmıştır. Sonuç: Mevcut klinik sonuçlarımız perinatal COVID-19 enfeksiyonunda asemptomatik veya hafif semptomlu vakalarda vertikal geçişin düşük olasılıklı olduğunu, postnatal dönemde uygun koruyucu ekipman kullanımının ve temas önlemlerinin bebeğe intrapartum ve postpartum bulaşı azaltmada oldukça önemli olduğunu desteklemektedir.
