Retrospective evaluation 298 arteriovenous fistula operations for hemodialysis access

Abstract
Objective: Patients with chronic renal disease for hemodialysis require permanent arteriovenous fistulas (AVFs) which have a good long term patency and low complication rate. The aim of this study was to evaluate our AVF experience and to evaluate the success rate and the complications in AVF operations performed for hemodialysis access.Material and Methods: We retrospectively reviewed 298 consecutive operations on 264 patients for hemodialysis access performed in our department between January 2007 and December 2012. Of the 264 cases, 162 were males (61%) and 102 were females (39%).The mean age was 58.8±6.4 years (range: 19-93). The non-dominant arm was chosen for the operation. The patients were analyzed for the type of the operation and the complications.Results: No vascular synthetic grafts were used in this series of 298 consecutive operations. Six-month patency rate for the fistulas were 94%. The complication rate was 14.42 %, and the most common complication was fistula thrombosis.Conclusion: All arteriovenous fistulas for chronic renal failure should aim a long duration of patency and productivity. Brescia-Cimino type distal arteriovenous fistula should be performed as the first choice for the vascular access. Native fistulas are reliable, safe and simple procedures with access sites often available both in the forearm and the upper arm.

Amaç: Kronik böbrek yetmezliği hastaları hemodiyaliz işlemi için düşük komplikasyon oranına sahip ve uzun süre açık kalabilen kalıcı arteriyovenöz fistüllere gereksinim duyarlar. Bu çalışmanın amacı, hemodiyaliz amaçlı arteriyovenöz fistül operasyonlarındaki deneyim, başarı oranı ve komplikasyonları değerlendirmektir.Gereç ve Yöntemler: Ocak 2007 ile Aralık 2012 tarihleri arasında hemodiyaliz amaçlı 264 hastada gerçekleştirilen ardışık 298 arteriyovenöz fistül operasyonunu retrospektif olarak değerlendirdik. Hastaların 162si erkek (%61) ve 102si kadındı (%39). Yaş ortalaması 58,8±6,4 yıl olup, en düşük yaş 19 ve en yüksek yaş 93tü. Operasyon için uygun olan nondominant kol seçildi. Değerlendirme operasyonun tipine, lokalizasyona ve komplikasyonlarına göre yapıldı.Bulgular: Bu 298 operasyonda hiç vasküler sentetik greft kullanılmadı. Altı aylık açık kalma oranı %94tü. Komplikasyon oranı (%14,4) olup, en sık görülen komplikasyon, fistülün trombozuydu.Sonuç: Kronik böbrek yetmezliği nedeniyle yapılan fistüllerdeki temel amaç, yeterli akım hızını sağlayan, uzun süreli açık kalan fistül oluşturmak olmalıdır. Kronik böbrek yetmezliği hastalarında uzun süre kullanımı kanıtlanmış Brescia-Cimino tipi distal girişimler, ilk seçenek olarak tercih edilmelidir. Nativ fistüller hem kolda hem de önkolda başarılı, güvenli ve kolay olarak gerçekleştirilebilirler.
