The diagnostic value of hematuria in genitourinary system trauma

Abstract

In this retrospective study, urine analysis of 139 patients with GUS trauma who were hospitalized in our clinic between 1983-1999 were investigated for a possible relationship between trauma severity and hematuiria. 119 of cases (86%) were blunt trauma and 20 of them (14%) penetrating. 69 injuries were at kidney, two at ureter, 11 at urinary bladder and 57 at uretra. In 41 patients, microscopic hematuria was found. In 23 cases out of 41 minor renal trauma, and in other 10 cases, major kidney injury was present. In remaining 8 patients, ureteral trauma was seen in 2, and major bladder trauma was present in 6 patients. Macroscopic hematuria was found in 27 cases. In 16 cases out of 27, minor renal trauma, and in other 6 cases, major renal trauma and in the remaining 5, major bladder injury was recognized. Urethrorrhagia was observed in 47 of our cases and all of them presented urethral injury. In the remaining 24 patients of our series, urine analysis were evaluated as normal in 14 patients and urine examples could not be obtained in other 10. In 10 of 14 patients presenting normal urine examples had minor renal trauma and other 4 presented major renal trauma. In 10 patients whom we could not obtained urine examples, we diagnosed complete urethral rupture. When we compare the results of urine analysis and the type of GUS trauma, we found a statistically significance between microscopic hematuiria and minor renal injury as well as urethrorrhagia and urethral rupture(p=0.02, p

Bu çalışmada 1983-1999 yıllan arasında GÜS yaralanması saptanan 139 hastanın idrar bulguları retrospektif olarak gözden geçirilerek, travmanın şiddeti ile olan ilişkisi araştırıldı. Yaralanma, olguların 119'unda (%86) künt, 20'sinde (%14) penetran travmaya bağlı oluşmuştu. Hastaların 69'unda böbrek, 2'sinde üreter, 11'inde mesane ve 57'sinde üretra yaralanması saptandı. Hastaların 41'inde mikroskopik hematüri görüldü. Bu hastaların 23'ünde minör ve 10'unda majör böbrek yaralanması, 2'sinde üreter, 6'sında majör mesane yaralanması saptandı. Makroskopik hematüri 27 hastada gözlendi. Bunların 16'sında minör ve 6'smda majör böbrek yaralanması ve 5'inde de majör mesane yaralanması izlendi. Kırkyedi olgumuzda üretroraji görüldü. Bu hastaların tümünde üretra yaralanması saptandı. Geriye kalan 24 hastanın 14'ünde idrar bulgusu normal bulunurken, 10 hastada idrar örneği alınamadı. İdrar bulguları normal olan bu 14 hastanın 10'unda minör böbrek, 4'ünde majör böbrek yaralanması bulunurken, idrar örneği alınamayan 10 hastaya tam üretra kopması tanısı koyuldu. İdrar analiz sonuçları ile genitoüriner organ yaralanması arasında yapılan istatistiksel değerlendirmede mikroskopik hematüri ile minör böbrek yaralanması ve üretroraji ile üretra yaralanması arasında anlamlılık saptandı (p=0,02, p
