Long-term esophageal pH monitoring in childhood gastroesophageal reflux

Abstract

Gastroesophageal reflux (GER) is seen frequently in childhood and may cause different symptoms. In this study, children with symptoms which were thought to be due to GER were evaluated by 24-hour esophageal pH monitoring and those with GER were given cisapride plus ranitidine treatment. Fifty patients (31 males, 62%), aged between 15 days and 16 years (mean 40.1 months, median 16 months), who were brought to our department with various complaints were enrolled into the study. The pH probe was placed into the lower esophagus as described before and 24-hour recordings were obtained (Mark HI, Synectics). The recordings were downloaded to a personal computer and reflux index, number of reflux episodes in 24 hours, number of reflux episodes longer than five minutes in 24 hours and duration of the longest reflux episode were determined using a special software (Gastrosoft). GER was diagnosed in 21 patients (42%) using these parameters. Fourteen patients (66.6%) out of 21 children with GER had vomiting. In addition to vomiting, two patients had hematemesis, two had abdominal pain, and one had recurrent lung infection. Apnea (2 patients), chest pain (2), abdominal pain (2) and cyanosis (1) were the main complaints. At the end of the treatment, improvement was observed in all patients by clinically and/or by pH monitoring. It is concluded that children with GER may come to the hospital not only with vomiting but also with different symptoms such as apnea, chest pain, hematemesis and abdominal pain.

Gastroösefageal reflu (GÖR) çocukluk çağında sık görülmekte ve çok değişik semptomlara neden olabilmektedir. Bu çalışmada değişik semptomlarla gelen ve klinik olarak GÖR düşünülen çocuklar 24 saatlik pH monitörizasyonu ile değerlendirilip reflu saptananlara sisaprid ve ranitidin tedavisi verildi. Son altı ay içinde değişik yakınmalarla bölümümüze başvuran ve yaşları 15 gün - 16 yıl (ortalama 40.1 ay, ortanca 16 ay) arasında olan 50 hasta (31 erkek, % 62) çalışmaya alındı. pH elektrodu önerildiği şekilde ösefagusa yerleştirildikten sonra 24 saatlik kayıtlar elde edildi (Mark III, Synectics) ve özel bilgisayar programı (Gastrosoft) kullanılarak reflu indeksi, en uzun reflu süresi, 24 saatteki reflu sayısı ve beş dakikadan uzun süren reflu sayısı bulundu. Bu parametrelere göre 50 hastanın 21'inde (% 42) GÖR saptandı. Reflu saptanan 21 hastanın 14'ünde (% 66.6) kusma yakınması vardı. Bunların ikisinde hematemez, ikisinde karın ağrısı, birisinde ise tekrarlayan akciğer enfeksiyonu kusmaya eşlik ediyordu. Kalan yedi çocukta apne, göğüs ağrısı ve karın ağrısı ikişer (% 9.5), siyanoz ise bir (% 4.9) hastada esas semptomdu. Tedavi sonrası kontrolü yapılan hastaların tamamında klinik ve/veya pH monitörizasyonu ile düzelme gözlendi. Sonuç olarak GÖR' si olan çocukların yalnız kusmayla değil, apne, göğüs ağrısı, hematemez ve karın ağrısı gibi değişik semptomlarla da gelebilecekleri gösterildi.
