Breast-feeding practices for 12 to 36 month-old children in an urban area in Kayseri

Abstract
This study was carried out in two urban health center areas to determine the knowledge, attitude and practices of women about breast-feeding. A questionnaire was given to 202 mothers attending a health center with children aged 12-36 months. The mean age of mothers was 26.7 ± 5.0. The vast majority of the mothers were housewives, 56.4 percent had completed primary school, and 75.7 percent had an average income level. Seven babies (3.5%) were never breast-fed; 96.5 percent of the children had been breast-fed at least one month. A small percentage (5.4%) were still breast-feeding. The mean duration of breast-feeding was 11.8 ± 6.5 months. Mothers who had breast-fed their children for six months or less accounted for 29.3 percent. It was determined that breast-feeding was not associated with mother's age, education level, type of family, economic status, gender of child, weight at birth or method of birth. The only relationship found was between breast-feeding and mother's employment. Housewives were likely to breast-feed their children more than employed women. According to logistic regression analysis of confounding factors with breastfeeding duration, only birth order of child and type of family demonstrated a clear relationship. Not being a first child increased the possibility of breast-feeding beyond six months. The possibility of breast-feeding a child beyond six months was also higher in large families when compared with the nucleus families. Over half (59%) of the mothers started breast-feeding in the first hour after delivery; 93.3 percent of the babies were given colostrum. The children who had not been breast-fed had significantly lower weight than children who had been breast-fed. There was no relationship between breast-feeding and the height of the baby. These results support the view that mothers should be encouraged to breast-feed their children for the first year of life with the addition of good quality weaning foods from about 4-6 months.

Bu çalışma 12-36 aylık çocuğu olan annelerin emzirme durumlarını ve etkileyen faktörleri belirlemek, emzirme ile ilgili bilgi düzeylerini saptamak ve bu konulara ilişkin öneriler sunmak amacıyla planlanmıştır Kentsel alandaki iki sağlık ocağı bölgesinde yapılan bu çalışmada sağlık ocağına başvuran 12-36 aylık çocuğu olan 202 anneye anket uygulanmıştır. Araştırma grubuna alınan annelerin yaş ortalaması 26.7 ± 5.0'dir. Büyük çoğunluğu ev hanımı, %56.4'U ilkokul mezunu, %75.7'si orta gelir düzeyine sahiptir. Emzirme yüzdesi %96.5 olup, annelerin %5.4'ü emzirmeye devam etmektedirler. Annelerden yedisi (%5.4) hiç anne sütü vermemiştir. Emzirme süresi ortalaması 11.8 ±6.5 aydır. Bebeğini altı ay ve daha az süre emzirenler %29.3'dür. Annenin yaşı, eğitimi, aile tipi, çocuğun cinsiyeti, doğum kilosu ve doğum şekli ile çocuğun emzirilme durumu arasında bir ilişki yokken, annenin çalışması açısından önemli bir ilişki vardır. Çalışan annelerde emzirme sıklığı daha düşüktür. Anne sütü verme süresini etkileyen faktörler ile ilgili yapılan lojistik regresyon analizi sonucunda çocuğun doğum sırası ve yaşanılan aile tipi ile emzirme süresi arasında önemli bir ilişki olduğu saptanmıştır. Çocuğun doğum sırasının artması ve geniş ailede yaşama çocukların altı ay ve daha uzun süre emzirme olasılıklarını artırmaktadır. Annelerin %59.0'ı bebeklerini doğumdan sonra ilk saat içinde emzirmişlerdir. Kolostrum verme yüzdesi %93.3'dür. Anne sütü alma ile çocuğun ağırlık durumu arasında önemli bir ilişki varken, boy durumu arasında bir ilişki bulunmamıştır. Bu çalışmanın sonuçlarına göre araştırma grubumuzda yer alan kentsel alanda yaşayan annelerin emzirmeye başlamaları konusunda önemli bir sorun yoktur. Temel sorun ek besinlere erken ve geç başlamadır. Her düzeydeki sağlık personeli ve özellikle ebelerin emzirmenin bilinçli olarak yapılması, doğru uygulanması ve ek besinlere zamanında başlanması konusunda annelere sürekli sağlık eğitimi yapması ve bu eğitimin medya tarafından desteklenmesi önerilmektedir.
