Cervical exenteration and gastric pull up for hypoharynx and cervical esophagus carcinoma

Abstract
Cervical Exenteration and Gastric Pull up for Hypo-pharynx and Cervical Esophagus Carcinoma The treatment of hypopharynx and cervical esophagus car¬cinoma is controversial. The aim of the study is to deter¬mine the status of cervical exenteration and gastric pull up procedure for hypopharynx and cervical esophagus carci¬noma and present our experiences with this procedure. We operated ten patients (seven voman, three men) for ex¬tensive larynx, hypopharynx and cervical esophagus carci¬noma between 1985-1995 at the Department of General Surgery of Cerrahpaşa Medical School. Medical records of these patients were evaluated retrospectively. Three pa¬tients had recurrent disease, two of them had radiotherapy, one had been treated by surgery and radiotherapy inittialy. According to postoperative staging one patient was in stage HA, one was in stage IIB where the others were in stage III. Incomplete resections were performed in two cases and complete resection in eight cases. Mean hospi-talization period was 25.2 (7-60) days. Postoperative com¬plications occured in 4 patients; anastomotic leakage (2 cases), pneumothorax (1 case) ARDS (1 case). One of the patients with anastomotic leakage died due to heamor-rhagefrom carotid artery after secondary cervical sepsis. Anastomotic stricture occured in the other patient with anastomotic leakage and he was treated with endoscopic dilatation successfully. Effective palliation can be achieved by cervical exentera¬tion and gastric pull-up procedure for carcinoma of the extansive larynx; hypopharynx and cervical esophagus de¬spite its higher morbidity and mortality. It can be consid¬ered as a curative operation for patients in early stage.

Hipofarenks ve servikal özofagus kanserlerinin tedavisi tartışmalıdır. Bu çalışmanın amacı hipofarenks ve servikal özofagus kanserlerinde servikal ekzenterasyon ve gastrik transpozisyon yönteminin yerini açıklamak; ve bu yöntem¬le öpere edilen olgularımızın sonuçlarını sunmaktır. CTF Genel Cerrahi ABD'nda 1985-1995 yılları arasında ekibimiz tarafından ekstansif larenks, hipofarenks ve ser¬vikal özofagus kanseri nedeniyle öpere edilen 7'si kadın, 3'ü erkek, ortalama yaşı 55, (22-71) olan 10 olgunun dosyalan retrospektif olarak incelendi. Olgularımızın 3'ünde de nüks tümöral hastalık vardı. Bunların ikisi inisiyal tedavi olarak radyoterapi, biri cerrahi+radyoterapi görmüştü; . Postoperatif TNM evrelendirmeye göre olgularımızın birisi evre 11-A; biri evre IIB; sekizi evre III, tümörlerdi. İki olguda inkomplet, 8 olguda komplet rezeksiyon gerçek¬leştirildi. Ortalama hospitalizasyon 25.2 (7-60) gündü. 4 olgumuzda postoperatif dönemde komplikasyon oluştu: anastomoz kaçağı (2 olgu), pnömotoraks (1 olgu), ARDS (1 olgu). Anastomoz kaçağı olan olgularımızdan biri ser¬vikal sepsise bağlı karotis arteri rüptürü sonucu eksitus olurken, diğer anastomoz kaçağı olgumuzda endoskopik dilatasyona cevap veren striktür gelişti. Mortalite oranı¬mız % 20'dir. (ARDS ve anastomoz kaçağı). Ameliyattan sonra ortalama survi 17.8 (7-108) aydır. Yüksek morbidite ve mortalitesine rağmen servikal ekzen¬terasyon ve gastrik transpozisyon, ekstansif larenks, hipo¬farenks ve servikal özofagus kanserlerinde effektif palyas-yon sağlayan, uygun evredeki olgularda da kür 'şansı veren radikal bir ameliyattır.
