Abstract
Is Cleansing of the Colon Necessary in the Surgical Treatment of Sigmoid Volvulus? Management of sigmoid volvulus is still controversial. The risk of anastomotic leakage is about twenty percent in emergent operations for volvulus. In this study, patients who underwent emergency resection and primary anas¬tomosis at one stage without intraoperative colonic lavage were compared to the patients in whom other surgical procedures were employed in terms of morbidity, mortali¬ty, hospital cost and workday loss. Forty eight consecutive patients operated on with diagnosis of sigmoid volvulus over a 5 year period were prospectively evaluated. There were 39 men and 9 women averaging 59.4 years of age (range, 18-79 years). Most of the patients whose main complaints were symptoms of mechanically intestinal ob¬struction were admitted to our hospital after more than 24 hours from initial symptoms During the operation, failure of arterial supply at sigmoid colon was determined in 18 patients (48.6 %). When resection with primary anastomo¬sis was performed in 18 patients at the same stage (Group one); in 16 patients, resection with the construction of a colostomy (Hartmann's Procedure) was made (Group two); in the remaining 14 patients first detorsion and then resection with anastomosis were done (Group 3). Among the first group patients 3 local and one systemic complica¬tion developed and in the second group patients two local complications and in the third group patients one systemic complication occured. One patient from the third group died (% 6.2). The duration of antibiotherapy, hospital stay, workday loss and treatment cost were significantly lower in the first group. In conclusion, this preleminary study suggests that resection with primary anastomosis at the same stage without intraoperative colonic lavage is a safe and relaiable procedure in the treatment of sigmoid volvulus.

Sigmoid volvuluslarda tedavi planı hala tartışmalıdır. Ge¬nel kabul gören görüş cerrahi tedavi yöntemidir. Acil şart¬larda yapılan ameliyatlarda anastomoz ayrışma riski % 20 dolayındadır. Bu çalışmada kolonik lavaj yapılmadan tek seansta rezeksiyon ve primer anastomoz yapılan hasta¬lar ile diğer cerrahi tedavi metodları uygulanan hastalar morbidite, mortalite, hastane maliyeti ve işgünü kaybı açı¬sından karşılaştırıldı. Kliniğimizde sigmoid volvulus tanısı ile başvuran 48 hasta üç guruba ayrıldı. Gurup l'de (n=18, % 37.5) kolon hazırlığı yapılmaksızın rezeksiyon ve aynı seansta primer anastomoz yapıldı. Gurup 2'de (n=16, % 33.) rezeksiyonla birlikte proksimale kolostomi (Hartmann prosedürü) ve daha sonra kolostomisi kapatıl¬ması uygulandı. Gurup 3'de (n=14, % 29.2) ilk seansta detorsiyon daha sonra elektif koşullarda kolon hazırlığı yapılarak rezeksiyon ve primer anastomoz uygulandı. Hastaların 39'u erkek, 9'u kadın olup, yaş ortalaması 59.4 idi (18-79 arasında). Başvuru nedenleri mekanik ileus semptomları olup, hastaların çoğu semptomların başlan¬gıcından 24 saatten fazla süren geçtikten sonra başvur¬muştu. Ameliyat esnasında 18 hastada (% 37.5) sigmoid kolonda beslenme bozukluğu tespit edildi. Birinci gurupta 3 hastada lokal bir hastada sistemik, ikinci gurupta 2 hastada lokal, üçüncü gurupta ise bir hastada sistemik komplikasyon gelişti.
