Abstract

Early Predictors of Posttraumatic Multiple Organ Failure Multipl Organ Failure (MOF) is the leading cause of post-traumatic deaths. Mortality risk is over 95 % at the later stages. It is important for the clinicians to diagnose and treat the syndrome before the irreversible stages are en¬tered. The aim of this study is to predict the high risk pa¬tients for MOF in the immediate posttraumatic period by using ISS and early shock indexes. In addition, we ques¬tioned whether easily measured indexes of cellular respi¬ration (lactate, base deficit) would improve this predicta¬bility. 45 trauma patients with an injury severity score (ISS) greater than 15, with an age greater than 16 years and who survived longer than 24 hours are studied pros-pectively. Age, sex, type of trauma ISS; systolic blood pressure, Glasgow Coma Scale (GCS), units of red blood cells transfused, base deficid and lactate levels are all re¬corded. MOF was scored after 48 hours of admission. The incidence of recorded parameters on the treshold val¬ues of MOF and non MOF group patients are found and the differences between these ratios are analyzed statisti¬cally. The following variables were identified as predic¬tors of MOF: age more than 55 years, ISS greater than or ' equal to 25, more than 6U of red blood cells transfused in the first 24 hours after admission, base deficit greater than 8 mEqIL and lactate greater than 2.5 mmollL. There was no difference in the percentage of patients with blunt mechanism of injury, preexisting disease or GCS less than 8.

Özet

Multipl Organ Yetmezliği (MOY), birden fazla organ veya sistemin fonksiyonunun bozulmasıdır. Çalışmamızın ama¬cı; bir yaralanma indeksine ve erken şok-belirteçlerine da yanılarak, posttravmatik erken dönemde MOY sendromu için yüksek riskli hastalan öngörebilmektir. Ayrıca hücre¬sel solunumun kolayca ölçülebilir indekslerinin (laktat, baz defisit) bu öngörüye katkısı olup olmadığını da araş¬tırmayı amaçladık. Çalışmamızda 16 yaşından büyük ve Yaralanma Şiddeti Puanı (ISS)>15 olanlar ve 24 saatten fazla yaşayan 45 hasta prospektif olarak değerlendirilmiştir. Yaş, cins, yaralanma şekli, yaralanma şiddeti puanı (ISS), sistolik kan basıncı, Glasgow Koma Skoru (GKS), ek hastalık, transfüze edilen kan miktarı, arterial kanda.baz defisit've laktat düzeyi kaydedilmiştir. MOY, başvuruyu takiben 48 saat sonra skorlanmıştır. Kaydedilen parametrelerin MOY gelişen ve gelişmeyen gruptaki hastalarda eşik değerlerde görülme oranları sap¬tanmış ve bu oranlar arasındaki farkların anlamlı olup olmadığı istatistiksel olarak değerlendirilmiştir. Yaş>55, ISS>25, ilk 24 saatte kan transfüzyonu >6 Ü, sistolik KB8 meqIL ve Laktat>2.5 mmoll L olmasının multipl organ yetmezliği risk faktörü olarak anlamlı parametreler olduğu bulunmuştur.
