Alkaline reflux gastritis and diagnostic problems

Abstract
Alkaline reflux gastritis which wis one of the most impor¬tant postoperative problems that may occur following gas¬tric surgery and may also be seen in non-operated pa¬tients, and its diagnostic problems were discussed. There were 90 patients in this study and 38 of these 90 patients were in the operative group (group I), 32 were in the non-operative symptomatic group (group II), and 20 were in the control group (group III). All the patients had hepato-biliary scintigraphy and in 46 patients whose reflux were diagnosed in scintigraphy, endoscopic biopsy were ap¬plied. In group I 74 % of the patients had reflux and 63 % had gastritis. While the reflux and gastritis were seen in all the patients who had Billroth-II+jejunojejunostomy no reflux and gastritis were determined in the patients who had proximal gastric vagotomy in these group. The rates of alkaline reflux and gastritis were 44 % and 25 % in group II and 20 % and 0 % in group III. It is concluded that selection of the operative method is very effective in prevention of alkaline reflux and gastritis, and since the symptoms are nonspesific the patients have to be evaluat¬ed with objective methods such as hepatobiliary scintigra¬phy and endoscopic biopsy in order to confirm the diagno¬sis and to plan management.

Gastrik cerrahiyi takiben ortaya çıkan postoperatif prob¬lemlerin en önemlilerinden biri olan ve operasyon geçir-memiş kişilerde de görülebilen alkalen reflü gastrit ve tanı problemleri tartışıldı. Çalışmaya alınan 90 hastanın 38'i operatif hastalar (grup I), 32'si non-operatif semptomatik hastalar (grup II) ve 20'si kontrol grubundan (grup III) oluşmaktaydı. Tüm hastalara hepatobilier sintigrafi, bu işlem sonucunda reflü saptanan 46 hastaya endoskopik biopsi uygulandı. Grup I hastaların % 74'ünde endoskopik olarak reflü ve % 63'ünde gastrit saptandı. Bu grupta Bill-roth-II+jejunojejunostomi uygulanan hastaların hepsinde reflü ve gastrit görülürken, .proksimal gastrik vagotomi uygulanan hastaların hiçbirinde bu bulgulara rastlanma¬dı. Grup II hastalarda alkalen reflü ve gastrit oranları sırasıyla % 44 ve % 25, grup III olgularda % 20 ve % 0 idi. Seçilen cerrahi yöntemin alkalen reflünün önlenme¬sinde etkili olduğu ve semptomlar nonspesifik olduğun¬dan dolayı kesin tanı koyup uygun tedaviyi planlayabil¬mek için hepatobilier sintigrafi ve endoskopik biopsi gibi objektif yöntemlerle değerlendirilmesi gerektiği kararına varıldı.
