Abstract

Fine Catheter Aspiration Cytology of the Peritoneum: Help in the Diagnosis of Acute Appendicitis? Fine catheter aspiration cytology of the peritoneum (FCACP) was pel formed on 72 cases suffering from pain in the right iliac fossa in order to evaluate the value of FCACP in the diagnosis of appendicitis. In two cases the access into peritoneum was impossible due to obesity and sufficient aspiration material could not be taken in four patients. No appendicitis was detected in six of 39 cases, whose cytology revealed positive results. In one of these six cases lymphoid hyperplasia of the appendix, in two of them salpingitis and in one of them renal colic was detect¬ed, while two of these cases were considered as nonspecif¬ic abdominal pain. Appendicitis was detected in four cas¬es, whose cytologic examinations were negative and in one case in whom peritoneal access was impossible. The sensitivity of FCACP for appendicitis was found as 86.8 % and the specificity as 82.3 %. The positive predic¬tive value of the test was 84.6 %, while negative predictive value was 84.8 %. Complications consisted of a stopped bleeding origination from mesenter vessels of the small in¬testine in one case and of a puncture of the small intestine in another case. Due to the positive and negative predic¬tive values that are not high enough, relative invasiveness of the method and possibility of complications: FCACP is not a method that can be performed primarily in the diag¬nosis of acute appendicits, but it may be helpfull in supp-porting the diagnosis of the patients with diagnostic diffi¬culties.

Akut apandisit tanısındaki değerini araştırmak amacıyla peritonun ince kateter aspirasyon sitolojisi (PİKAS) sağ fossa iliakasında akut ağrı yakılmasıyla başvuran 72 has¬taya uygulandı. İki hastada şişmanlık nedeniyle periton boşluğuna girilemedi. Dört olguda yeterli materyal alına¬madı. Sitolojisi pozitif 39 olgudan altısında apandisit yoktu. Bu altı olgudan ameliyat edilen birinde apandiste lenfoid hiperplazi, iki olguda salpenjit, birinde renal kolik vardı, ikisi non-spesifik karın ağrısı olarak değerlendiril¬di. Sitolojisi negatif olan dört, peritona ulaşılamayan bir olguda apandisit saptandı. PİKAS'ın akut apandisit için duyarlılığı % 86.6, özgüllüğü % 82.3, pozitif tahmin değe¬ri % 84.6, negatif tahmin değeri % 84.8 idi. Komplikasyon olarak bir olguda ince barsak mezenter damarlarından kaynaklanan durmuş kanama, bir olguda ince barsak punktürü vardı. PİKAS pozitif ve negatif tahmin değerinin yeterince yüksek olmaması, göreceli invazivliği ve kompli¬kasyon gelişme olasılığı nedeniyle akut apandisit tanısın¬da öncelikli bir yöntem olamaz. Ancak tanı güçlüğü olan olgularda tanıyı desteklemede yardımcı olabilir.
