Implantable venous port application

Abstract
SUMMARY Implantable Venous Port Application Since 1970s, the need for intravenous lines and fragility of périphérie vessels in cancer patients lead to.progressions in permanent "implantable venous ports". Venous port was applied to 63 patients between January 1995 and December 1997. 29 of these patients were female and 34 were male with a mean age of 51 (11-78). Port indications were gastrointestinal system cancers in 40 (63 %) cases, endocrine system cancers in 10 16 %) cases, other system cancers in 12 (19 %) cases and short bowel syndrome in 1 (2 %) case. Ports were implanted in the operation room under local anesthesia. Implantation site was external jugular vein in 26 cases, cephalic vein in 25 cases, subclavi-an vein in 11 cases and internal jugular vein in 1 case. Patients were discharged on the same day and called for control 1 week later. Complications were seen in 7 (11 %) cases. Ports were taken out in 3 cases with catheter malposition and 1 case of skin necrosis. Implantable venous ports must be considered in patients requering long-term IV therapy, especially cancer patients because of high quality of life it provides and the ease of appliance.

Kanserli hastalarda damaryolu ihtiyacı ve periferik damarların kısa sürede kullanılamaz hale gelmesi 1970'ler-den itibaren kalıcı "implantabl venöz porfların gelişmesine yol açmıştır. Ocak 1995-Arahk 1997 tarihleri arasında 63 hastaya venöz port uygulandı. Hastaların 29'u kadın, 34'ü erkek olup ortalama yaş 51 (11-78) dir. Port uygulanma nedeni 40 (% 63) olguda gastrointestinal sistem kanseri, 10 (% 16) olguda endokrin sistem kanseri, 12 (% 19) olguda diğer sisteme ait kanser ve bir (% 2) olguda kısa barsak sendromudur. Portlar ameliyathane kofullarında, lokal anestezi altında implante edildi (takıldı). Kateter giriş yeri olarak 26 olguda v.jugularis eksterna, 25 olguda v.cephalica, 11 olguda v.subclavia ve bir olguda v.jugularis internal kullanıldı. Hastalar günübirlik taburcu edilip bir hafta sonra kontrole çağırıldı. Olguların 7'sinde (% 11) komplikasyon görüldü. Kateter malpozisyonlu üç ve cilt nekrozu gelişen bir olguda portların çıkartılması gerekmiştir. İmplantabl venöz port kullanımı, başta kanser hastaları olmak üzere uzun süreli intravenöz tedavi gerektiren hastalarda uygulama kolaylığı ve hastanın yaşam kalitesini arttırması nedeniyle akılda bulundurulmalıdır.
