Abstract

Pheochromocytoma may be present in more than one localization in some cases. Although there was a single mass of pheochromocytoma in the preoperative computerized tomography (CT) and II3I-MIBG scan, two seperate chromaffin cell tumors were removed during the operation. Retrospective evaluation of the preoperative CT proved that a secondary mass was in fact present. On the other hand, similarly, it was postoperatively recognized that the uptake at the preoperative I131-MIBG scan also contained both of the mass removed in the surgery. Thus, preoperative diagnostic investigations should be carefully evaluated considering that pheochromocytoma can occur in more than one localization. Surgical exploration should also be meticulous to look for possible additional pheochromocytomas.

Özet

Feokromositoma, kromafin hücrelerden kaynaklanan ve hipertansiyon, yüzde kızarma (flushing) gibi bulgularla karakterize bir patolojidir. Kliniğe bu bulgularla başvuran hastalarda, 24 saatlik idrarda vanilil mandelik asit (VMA), metanefrin düzeyi saptanarak tanı konabilmektedir. Tümörün lokalizasyonu, BT, manyetik rezonans (MR), I131-MIBG sintigrafisi gibi tetkiklerle belirlenmekte, daha sonra tedavi amacıyla cerrahi girişim yapılmaktadır. Tümör ya da tümörlerin yerleşiminin doğru saptanması, cerrahi tedavinin güvenle ve tam olarak yapılmasını sağlayacaktır.
