Protective effect of intraductal and systemic catalase treatment in hydrogen peroxide-induced experimental acute pancreatitis

Abstract
It was known that free oxygen radicals (FOR) have a role on the parenchymal damage observed in acute pancreatitis. However, the mechanism by which FOR induced pancreatic damage has yet to be established. Specific radical scavengers used in FOR-induced acute pancreatitis may help to clarify the mechanism of damage in pancreas. In this experiment, we studied the protective effect of catalase in histologic and biochemical changes in H2O2 induced acute pancreatitis. The bile-pancreatic duct (BPD) was cannulated close to the liver in Wistar Albino rats and perfused with normal saline in group 1, H2O2 in group 2, H2O2+intraductal catalase in group 4. In group 3, prophylactic catalase enzyme was administered intravenously 1 hour prior to intraductal H2O2 perfusion. Although no pathological changes was found in group 1, marked pancreatitis was encountered in group 2 which was perfused with H2O2. Intravenous catalase administration did no improvement in gross and microscopic changes in pancreas and, malondialdehyde and amylase levels. On the other hand, intraductal perfusion of catalase did significantly decrease the malondialdehyde and amylase levels and prevent macroscopic edema formation. But, intraductal catalase perfusion did not prevent histologic changes including necrosis. As a result, intraductal H2O2 perfusion induced acute ancreatitis in rats although intraductal catalase perfusion did significantly decrease the amlylase and MDA levels, it had no protective effect on tissue necrosis. This finding suggests that a mechanism other than the toxic effect of H2O2 may play role on the development of tissue necrosis occurred in H2O2-induced acute pancreatitis.

Serbest oksijen radikallerinin (SOR) akut pankreatitte histopatolojik değişikliklerin oluşmasında rol oynadığı bilinmektedir. Ancak bu hasarın hangi mekanizma ile meydana geldiği ortaya konamamıştır. SOR ile oluşturulan akut pankreatit modelinde, spesifik radikal temizleyiciler ile yapılacak çalışmalar, SOR'in etki mekanizmasını açıklamaya yardımcı olabilir. Bu çalışmada, H2O2 ile oluşturulan akut pankreatitte ortaya çıkan histolojik ve biyokimyasal değişikliklerin katalaz enzimi ile hangi boyutta önlenebileceğini araştırmayı amaçladık. Bu çalışmada Wistar Albino tipi ratların ana pankreas kanalı karaciğer hilusu düzeyinde kateterize edilerek duodenuma doğru grup 1'de serum fizyolojik, grup 2'de H202, grup 4'de H2O2+katalaz ile perfüze edildi. Grup 3'de intraduktal H2O2 perfüzyonundan 1 saat önce proflaktik intravenöz katalaz verildi. Serum fizyolojik ile perfüze edilen ratlarda herhangi bir patoloji saptanmazken, H2O2 ile perfüze edilenlerde belirgin pankreatit gelişti. Proflaktik intravenöz katalaz verilimi makroskopik ve mikroskopik bulgularda, doku malondialdehit ve serum amilaz düzeylerinde bir düzelme yapmazken, intraduktal katalaz ödem oluşumunu engelledi, malondialdehit ve amilaz düzeylerini istatistiksel olarak azalttı (p
