Abstract

The Early Results of the Patients with Rectal Adenocar-cinoma Treated with Postoperative Chemoradiotherapy In randomized trials it has been shown that, the combina¬tion of postoperative chemotherapy and radiation signifi¬cantly improved the local tumor control, overall and dis¬ease-free survival rates in patients with rectal carcinoma. In Radiation Oncology Department of Cerrahpaşa Medi¬cal School a prospective, pilot postoperative chemoradio¬therapy study was opened for to evaluate the treatment toxicity and to decide the treatment policy in patients with rectal adenocarcinoma who were surgically treated. In this study, the early results and treatment toxicity of 29 patients with Modified Aster Coller stage B2, B3, C2, C3 rectal adenocarcinoma treated according to the protocol are reported. 5-flurouracil 450 mglm2lday was given on first 3 and last 3'days of radiotherapy with infusion lasted for 3 hours. In an hour after the completion of the infu¬sion, patients were irradiated. Chemotherapy with 5-flurouracil 425 mglmllday intravenous (IV) bolus andfol-nic acid 20 mglm2lday Dl-5, monthy was lasted 6 months after completion of radiotherapy. Patients who were treat¬ed with low-anterior resection had pelvic irradiation and patients treated with abdominoperineal resection had pel¬vic and perinea! irradiation. The total 5040 cGy tumor dose (TD) was given with 180 cGy perday and as 5 treat¬ments per week. The median follow-up was 21 months^(6.~ 35 months). The local control rates for I and 2 years were % 88 and % 82 respectively. The disease free survival rates for the same periods were % 84 and % 58 and the overall survival rates were % 96 and % 80 (Kaplan-Meier). Early side effects were evaluated according to the Radiation Therapy Oncology Group (RTOG) morbitidy scale and no grade 3 and 4 toxicity was observed. Three (10 %) patients had grade 2 leukopenia, 10 (35 %) pa¬tients had grade I and 3 diarrhea and 9 (31 %) patients had grade 2 radiodermatitis. Fourteen (48 %) patients had median 5 days of interval during radiotherapy be¬cause of the early side effects (2-12 days). As a conclu¬sion: although this treatment model is tolerable with mini¬mal side effects and promising overall survival rates, it is found out that disease-free interval rate for 2 years is short when compared with the other studies.

Rektum kanserinin adjuvan tedavisinde postoperatif kemo-radyoterapinin sağkalım, hastalıksız sağkalım ve lokal kontrolü anlamlı derecede arttırdığı randomize çalışmalar ile gösterilmiştir. Bunun sonucunda Cerrahpaşa Tıp Fa¬kültesi Radyasyon Onkolojisi Anabilim Dalında, Modifiye Astler Coller (MAC) evreleme sistemine göre evre B2,B3, C2, C3 rektum adenokanserli, cerrahi ile tedavi edilmiş olgularda, postoperatif kemoradyoterapinin tedavi toksisi-tesinin incelenmesi ve daha sonraki tedavi politikasının belirlenmesi amacı ile pilot, prospektif bir çalışma baş¬latılmıştır. Bu çalışmada, hazırlanan-protokole göre ke¬moradyoterapi ile tedavi edilmiş 29 olgunun erken sonuç¬ları ve tedavi toksisitesi değerlendirilmiştir. Olguların tü¬müne cerrahi sonrası radyoterapi ve eş zamanlı kemote-rapi başlanmıştır. Kemoterapi olarak 5-fluorourasil 450 mglmllgün radyoterapinin ilk üç ve son üç günü 3 saatlik infüzyonlar ile uygulanmış ve kemoterapi bittikten 1 saat sonra hastalar radyoterapiye alınmışlardır. Radyoterapi, sonrası 6 ay boyunca, 5-fluorourasil 425 mglmllgün int-ravenöz (İV) bolus ve folinik asit 20 mglmllgün IV bolus 1-5 günler, ayda bir olmak üzere devam etmiştir. Radyote¬rapi low-anterior rezeksiyon yapılan olgularda pelvis alanını, abdominoperineal rezeksiyon yapılan olgularda ise pelvis ve perine alanını içermektedir. Toplam 5040 cGy tümör dozu (TD), 180 cGy'lik fraksiyonlarla, haftada 5 fraksiyon ile uygulanmıştır. Yaşayan olgularda medyan takip 21 aydır (6-35 ay). Sonuçlar değerlendirildiğinde lo¬kal kontrol oranları I ve 2 yıl için sırası ile % 88 ve % 82, hastalıksız sağkalım oranlan aynı süreler.için % 84 ve % 58, sağkalım oranları ise % 96 ve % 80 olarak bulunmuştur (Kaplan-Meier). Tedavi toksisitesi olarak erken yan etki¬ler değerlendirilmiştir. Radiation Therapy Oncology'Gro-up (RTOG) morbidite tablosuna göre değerlendirme ya¬pıldığında hiç bir olguda grade 3 ve 4 toksisite gözlenme¬miştir. Erken yan etki olarak 3 (% 10) olguda grade 2 lökopeni 10 (% 35) olguda grade 1,2 diyar e ve 9 (% 31) olguda grade 2 cilt reaksiyonu kaydedilmiştir. Sonuç ola¬rak rektum adenokanserli olgularda uyguladığımız bu kombine tedavi programı tolere edilebilir ve toplam sağ¬kalım oranlan ümit verici olmakla birlikte 2 yıllık hasta¬lıksız sağkalım oranının diğer kombine tedavi uygulanan çalışmalara göre daha kısa olduğu görülmüştür.
