Effectiveness of extended lymph node dissection in curable gastric carcinoma

Abstract
Although prolonged survival rates have been reported in the limited prospective and mostly retrospective and non-radomised trials, there has been no concept on the extention of lymphadenectomy at the surgical management of patients with curable gastric cancer. In this prospective non-randomised trial, survival benefit of lymph node dissection is assessed on 52 patients who underwent curative surgery at the 4th Surgical Clinic Ankara Numune Training and Research Hospital between October 1991 and June 1998. All patients are followed up regularly. Mean follow up was 40 (range 13-86) months in surviving patients. Mean age was 61.3 (range 42-82) years and 78.8 % of the patients were males. Postoperative complication and mortality rates were 24.1 % and3.4 % in D1 group and 30 %, 13 % in D2 group, respectively. 1, 3 and 5 years survival rates of Dl lymph node dissection group were 55 %, 33.5 % and 33.5 %, respectively; and were 86.5 %, 70.6 % and 70.6 % respectively among D2 lymph node dissection group. D2 lymph node dissection is a reliable surgical choice in the surgical management of curable gastric adeno cancer patients with no significant adverse effect on postoperative morbidity and mortality rates with a significant survival benefit.

Sınırlı sayıdaki prospektif ve daha çok retrospektif/non-randomize çalışmalarda D2 lenf nodu disseksiyonu ile daha yüksek sağ kalım oranları bildirilmesine rağmen potansiyel kurabil mide kanserlerinde uygulanacak cerrahi tedavinin genişliği ile ilgili tam bir fikir birliği sağlanamamıştır. Bu klinik prospektif non-randomize çalışmada Ekim 1991-Haziran 1998 tarihleri arasında Ankara Numune Eğitim ve Araştırma Hastanesi 4. Cerrahi Kliniğinde küratif ameliyat yapılan 52 hasta değerlendirilerek lenf nodu disseksiyonunun sağ kalım üzerine etkisi incelendi. Hastaların tümü düzenli olarak takip edildi. Yaşayan hastalarda ortalama takip süresi 40 (13-86) aydı. Ortalama yaş 61.3 (42-82) olup hastaların % 78.8'i erkekti. Postoperatif komplikasyon ve mortalite oranları D1 grubunda % 24.1, % 3.4 iken D2 diseksiyon grubunda % 30, % 13 idi. D1 lenf nodu diseksiyonu yapılan grupta 1,3,5 yıllık sağ kalım oranları sırasıyla % 55, % 33.5 ve % 33.5 iken, D2 lenf nodu diseksiyonu yapılan grupta % 86.5, % 70.6, % 70.6 olarak tespit edildi. Kürabil mide adenokanserinin cerrahi tedavisinde D2 lenf nodu diseksiyonu güvenilir bir ameliyat olup postoperatif morbidite ve mortalite oranlarında ciddi bir artış sağlamadan yaşam süresini belirgin olarak artırmaktadır.
