Levothyroxine suppressive therapy for solitary thyroid nodules

Abstract

In this prospective randomized study, we examined the effects of levothyroxine suppressive therapy on volume of solitary thyroid nodules. Forty euthyroid patients with solitary thyroid nodules which were diagnosed by palpation, ultrasonography and scintigraphy and were benign cytologically were evaluated. Twenty patients from this group received levothyroxine and the others (control group) didn't. Clinical, biochemical and sonographic examination was performed at 3rd, 6th and 12th months. Fine needle aspiration cytology was performed every 6 months. The volumes of three nodules (% 15) were reduced with levothyroxine treatment in ratio more than % 50. The volumes of three nodules (% 15) enlarged in control group. No differences were found in the volumes of nodules at the beginning arid at the end of study between these groups statistically (p>0.05). These results suggest that TSH suppressive therapy with levothyroxine in patients with solitary thyroid nodules was not effective in reducing nodule volumes.

Özet

Bu çalışma, benin tiroid nodüllerin tedavisinde TSH supresyonunun etkinliğini araştırmak amacı ile randomize ve prospektif olarak planlandı. Malignite riski taşımayan, daha önceden tiroid hormon tedavisi görmemiş, ultrasonografide soliter nodulu bulunan, ince iğne aspirasyon biopsisi (İİAB) benin gelen, sintigrafide soğuk nodulu bulunan ve tiroid fonksiyon testleri normal sınırlar içinde olan 40 olgu çalışmaya alındı. Olgular yirmişerli iki gruba (kontrol ve tedavi grupları) ayrıldı. Tedavi grubundaki olgulara 100 mg/gün Levotiroksin verildi. Tüm olgular 3., 6. ve 12. aylarda kontrole çağrılarak ultrasonografik ölçüm ve tiroid fonksiyon testleri tekrarlandı. Altı ay aralarla İİAB tekrarlandı. Levotroksin ile tedavi edilen gruptan 3 (% 15) olguda tiroid volümünde % 50'den fazla azalma ve kontrol grubunda üç (% 15) olguda nodülde büyüme saptandı. Ancak, her iki grupta da çalışma başlangıcı ile çalışma sonunda ölçülen nodul volümleri arasında anlamlı farklılık yoktu (p>0.05). Sonuç olarak, soliter tiroid nodüllerinin yok edilmesi veya küçültülmesinde levotiroksin supresyon tedavisinin yetersiz kaldığını düşünmekteyiz.
