Radical management of pilonidal sinus disease by excision and primary closure under local anesthesia

Abstract

Between January 1998-May 1999, a retrospective trial was performed in 90 patients operated for pilonidal disease under local anesthesia. All of the patients were men. Median age of the patients whose were operated for excision and primary closure was 22 (20-39). Catgut suture, electrocoagulation and drain were not used in any operation. Postoperative wound infection was seen in 7 (% 7.7) and recurrence was seen in 2 (% 2.2) patients. In our trial we saw that wound healing was very fast, postoperative wound dressing was required approximately 5 (3-18) times, skin sutures were taken off in 14th (12-16) day and the patient could return to their normal activity in 3 weeks. As a conclusion, pilonidal disease in active infective period should be treated by surgical drainage and medical therapy. After healing period "excision and primary closure operation" can be performed. We intent that this is the more valuable operation, because early wound healing period makes patient's life more comfortable and causes least work power loss.

Ocak 1998-Mayıs 1999 tarihleri arasında, pilonidal sinüs nedeniyle lokal anestezi altında ameliyat edilen 90 hastada retrospektif çalışma yapıldı. Hastaların hepsi erkek olup yaş ortalaması 22 (20-39) idi. Hastalara lokal anestezi ile eksizyon ve primer kapama ameliyatı uygulandı. Hiçbir hastanın ameliyatında katgüt sütür, elektrokoter ve dren kullanılmadı. Postoperatif dönemde 7 hastada (% 7.7) yara enfeksiyonu, 2 hastada (% 2.2) nüks görüldü. Çalışmamızda hastaların yara iyileşmesinin çok hızlı olduğu, postoperatif dönemde ortalama 5 (3-18) kez pansuman gerektiği dikişlerinin ortalama 14. (12-16) günde alındığı ve aktif yaşamlarına üç hafta içinde dönebildikleri gözlemlendi. Pilonidal sinüs aktif enfeksiyon döneminde cerrahi drenaj ve medikal yöntemlerle tedavi edilmeli, enfeksiyon iyileştikten sonra "eksizyon ve primer kapama" ameliyatı uygulanmalıdır. Bu cerrahi yöntemin, erken dönemde yara iyileşmesine bağlı olarak hastaya daha iyi konfor sağlaması ve sonuçta iş gücü kaybının en az olması nedeniyle, kıymetli bir yöntem olduğuna inanmaktayız.
