Truncal vagotomy-antrectomy and gastroduodenostomy in elective surgical treatment of complicated duodenal ulcer

Abstract
Today, surgical interventions regarding duodenal ulcer are only attributable to the complicated cases. Emergency interventions are needed in perforation and massive hemorrhage. In cases of previous hemorrhagic attacks, severe gastric outlet obstruction and intractability, elective surgical interventions are applied. There are mainly 3 methods used in elective surgical therapy of complicated duodenal ulcer as proximal gastric vagotomy (PGV), Truncal vagotomy + drainage (TVD) and truncal vagotomy + antrectomy (TVA). In this study we presented 36 cases with long term follow up s in which we applied TVA + gastroduodenostomy (GD) according to our preference. Operations are done by us between 1983-1998 in Cerrahpaşa Medical Faculty, Department of General Surgery. Our series consisted of 31 male (86.1 %) and 5 female (13.8 %) patients. Ages of the patients were ranging between 18 and 67 with a mean age of'38.89±1.94 years. Indications for operation were recurrent hemorrhage causing blood transfusion in 12 cases (33.3 %), severe gastric outlet obstruction in 8 cases (22.2 %) and presence of in tractable ulcer in 16 cases (44.4 %). There were neither mortality nor recurrence in our series. Mean follow up period was 8.33±0.75 years. In 36 cases of which long term follow up was done periodically, 28 (77.7 %) were in group of Visick I, 8 (22.2 %) were in group of Visick 11. There were no patients in groups of Visick III and Visick IV. We believe that in cases preparing for elective surgery for complicated duodenal ulcer, TVA+GD is a safe and curative procedure.

Duodenal ülsere yönelik cerrahi girişimler günümüzde sadece komplike olgularda gündeme gelmektedir. Perforasyon ve abandan kanamalarda acil girişim zorunluluğu doğar. Geçirilmiş kanama atakları, ciddi mide çıkış obstrüksiyonu ve intraktabilite gibi durumlarda ise elektif girişim uygulanır. Komplike duodenal ülserin elektif cerrahi tedavisinde Proksimal Gastrik Vagotomi (PGV), Trunkal Vagotomi + Drenaj (TVD) ve Trunkal Vagotomi + Antrektomi (TVA) olmak üzere başlıca üç yöntem kullanılmaktadır. Bunlar arasında kişisel tercihimiz olan TVA + Gastroduodenostomi (GD) işlemini uyguladığımız 36 hasta, uzak takipleri ile sunulmaktadır. Ameliyatlar, 1983-1998 yıllan arasında Cerrahpaşa Tıp Fakültesi Genel Cerrahi Anabilim Dalı'nda tarafımızdan gerçekleştirilmiştir. Seride, 31 (% 86.1) erkek, 5 (% 13.8) kadın hasta bulunmaktadır. Yaş ortalaması 38.89±1.94 olup 18-67 arasında değişmektedir. Ameliyat endikasyonu 12 (% 33.3) olguda transfüzyon gerektiren mükerrer kanama, 8 (% 22.2) olguda ciddi mide çıkış obstrüksiyonu, 16 (% 44.4) olguda ise inatçı ülser varlığıdır. Serimizde mortalite ve nüks bulunmamaktadır. Ortalama takip süresi 8.5510.75 yıldır. Uzak takipleri düzenli olarak yapılan 36 olgunun 28 (% 77.7)'i Visick I gurubunda, 8 (% 22.2)'si Visick I gurubunda yer almıştır. Visick III ve IV gurubu hasta bulunmamaktadır. Komplike duodenal ülser nedeniyle elektif cerrahi düşünülen olgularda TVA+GD işleminin güvenle yapılabilecek küratif bir ameliyat olduğu inancındayız.
