Esophageal variceal bleeding due to portal hypertension: Endoscopic sclerotherapy as the first step treatment modality

Abstract
Variceal bleeding from the esophagus is an important cause of mortality and morbidity in children with portal hypertension. 41 intrahepatic and 28 extrahepatic, total of 69 portal hypertension cases were evaluated in our department since 1990. The period is limited to 1990 because one of the authors. According to Child's classification, 49 cases were included in Child A; 16 cases were in Child B and 4 of the cases were in Child C group at admission. In Cerrahpasa protocol; endoscopic sclerotherapy is performed in all portal hypertensive patients and the diagnosis is achieved directly by diagnostic laparoscopy and liver needle biopsy. Besides the routine ultrasonographic and Doppler ultrasonographic examination; abdominal tomography, perrectal scintigraphy, digital angiography and venous phase imaging techniques are also performed in some of the patients when needed. Sugiura operation was performed in 9 patients who presented with recurrent bleeding episodes despite the sclerotherapy, and who for this reason had to stay at the hospital for long durations. After the operation 8 of the patients did not bleed again but the last one died because of hepatic insufficiency. Liver transplantation was performed in two patients who were in Child C class. One died short after the operation because of primary nonfunction and the other was lost because of sepsis. Two cases in this series did not follow up the planned sclerotherapy programme and were lost due to massive bleeding at the terminal stage. Total mortality rate in this series is found to be 7 %, (5/69). Our clinical approach in the portal hypertensive cases presenting with bleeding is to stabilize the patient with conservative nonoperative measures and to perform the endoscopic sclerotherapy procedure after 3 days without bleeding. As a result endoscopic sclerotherapy, as presented herein Cerrahpaşa series, decreases the'need far additional surgical interventions in portal hypertensive children.
Özet
Portal hipertansiyon (PHT) ve buna bağlı gelişen özofagus varis kanamaları çocuk yaş grubunda halen ciddi bir mortalite ve morbidite nedenidir. Kliniğimizde,1990-l999 yıllan arasında, 41 intrahepatik ve 28 ekstrahepatik olmak üzere toplam 69 olgu PHT tanısıyla takip ve tedavi edilmiştir. Child sınıflaması gaz önüne alındığında 49 olgu Child A, 16 olgu Child B, 4 olgu Child C grubuna aittir. Portal hipertansif hastada Cerrahpaşa protokolüne göre tüm olgulara endoskopik skleroterapi uygulanmış; işlem genel anestezi altında, flexible endoskopla varis etrafına ve varis içerisine % 1 aethoxysclerol enjeksiyonu şeklinde uygulanmıştır. Hastalara laparoskopi ve karaciğer iğne biyopsisi'de yapılarak tanıya doğrudan ulaşılmaya çalışılmıştır. Rutin ultrasonografi ve Doppler ultrasonografi incelemelerinin yanısıra olguların bir kısmında abdominal tomografi, perrektal sintigrafi, dijital anjiografi ve venöz faz görüntüleme yöntemlerinden de yararlanılmıştır. Takip süresi içerisinde, kanama atakları sık olan ve uzun süre hastanede kalmayı gerektiren 9 olguda, cerrahi yöntem olarak Sugiura operasyonu uygulanmıştır. Bunlardan 8 tanesinde kanama tekrar etmemiş, 1 olgu ise karaciğer yetmezliği ile kaybedilmiştir. Child C sınıfında olup karaciğer nakli yapılan 2 olgu, sırasıyla donör karaciğer yetmezliğine bağlı primer nonfonksiyon ve sepsis nedeniyle kaybedilmişlerdir. Diğer 2 olgu planlanan skleroterapi programına uymayarak terminal dönemde kanama ile kaybedilmişlerdir. Tüm seride mortalite oranı 7 %, (5/69)'dur. Kanama ile başvuran PHT olgularında klinik olarak yaklaşımımız konservatif nonoperatif yöntemlerle stabilizasyonun sağlanması, ilk 3 günün kanamasız geçmesi, elektif şartlarda endoskopi ve skleroterapi şeklindedir. Sonuç olarak endoskopik skleroterapi yaklaşımı; PHT saptanmış çocuklarda, sunulan Cerrahpaşa serisinde görüldüğü üzere, ek cerrahi girişim gereksinimini azaltmaktadır.
