Acute thyroiditis with cutaneous fistulization

Abstract

An 85-year-old-male patient applied to surgery outpatient clinics with a painless mass in the anterior neck fistulized to skin. Preoperative radiological studies and fine needle aspiration revealed an acute inflammation in thyroid gland. Left total thyroidectomy was performed. Pathologic examination confirmed preoperative diagnosis as acute inflammation and granulation tissue. Postoperative course was unremarkable. The patient is under follow up for two years without any evidence of disease. Presentation of acute thyroiditis with a skin fistula is very uncommon. When there is a skin fistula overlying a neck mass in an elder patient, besides the common occurrence of a high grade carcinoma acute thyroiditis should also be considered in differential diagnosis.

Akut süppüratif tiroidit (AST) tiroid bezinin nadir görülen yangısal hastalığıdır. Genelde, hemen öncesinde geçirilmiş üst solunum yolu enfeksiyonu veya farenjit vardır. Yakın zamanda piriform sinüs fistülü ile beraber görüldüğü bildirilmiştir O. Biz, zeminde anomalisi bulunmayan deriye fistülize olmuş bir akut tiroidit olgusunu çok nadir görüldüğü ve karsinom ile ayırıcı tanısı açısından önemli olduğu için bildirmeyi uygun bulduk.
