Abstract

Anal fistula is a difficult surgical problem that is recorded worldwide. Treatment of that pathology, especially its complex types like horseshoe fistula has a high rate of complication. We discuss a case of horseshoe fistula, which is complicated with cysts located in the bilateral gluteal regions and extended nearly up to the middle part of the right thigh, after having an inadequate surgical intervention four years ago. Those cystic masses with their fibrotic elongation toward anus was excised successfully. The patient was discharged on 7th day and had no any later complication at his one year follow up period. Treatment of fistula must be done with care by experienced surgeons otherwise has a high rate of complication.
