Kutanöz lenfomalar: Derleme

Abstract

Skin may be affected by non-Hodgkin lymphomas, and it is the second most frequently involvedextranodal organ, after the gastrointestinal tract. Cutaneous lymphomas may originate from T, B,or NK lymphocytes. Diagnosis is difficult, and knowledge of these diseases is important to ensuretheir detection and adequate treatment and follow-up. Clinical picture is heterogeneous, and his- tology is essential to confirm the diagnosis. The classification is given by the correlation betweenclinical findings, histology and immunophenotyping. Therapeutic options include skin-directedtherapies, immunomodulatory or low-dose immunosuppressive drugs. Rare cases of aggressivedisease require systemic multidrug chemotherapy. The purpose of this review is to describe theontogeny of T, B and NK lymphocytes, and to detail the most accepted classification of cutaneouslymphomas, proposed by the World Health Organisation and European Organisation for Researchand Treatment of Cancer. Treatment of cutaneous lymphomas will be briefly discussed.

Non-Hodgkin lenfomalarda cilt etkilenebilir ve gastrointestinal sistemden sonra en sık ikincietkilenenen ekstranodal organ deridir. Kutanöz lenfomalar T, B veya NK lenfositten kökenalabilir. Tanı konulması zordur ve tanınmaları ile yeterli şekilde tedavi olup izleme alınmaları içinhastalığın iyi bilinmesi gerekir. Klinik tablo heterojen olup, tanının doğrulanması için histolojiönemlidir. Sınıflandırma klinik bulgular, histoloji ve immünfenotiplemedeki korelasyon ileyapılır.Tedavi seçenekleri cilde yönelik tedaviler, immünmodülatör veya düşük dozimmünsupresif tedavileri içerir. Agresif seyirli nadir vakalarda sistemik çok ilaçlı kemoterapigerekebilir. Bu derlemenin amacı T, B, ve NK lenfositlerinin ontogenisini tanımlamak ve DünyaSağlık Örgütü ile Avrupa Kanser Araştırma ve Tedavi Örgütünün kutanöz lenfomalar içinönerdiği en çok kabul gören sınıflandırmayı detaylandırmaktır. Kutanöz lenfomaların tedavisi dekısaca tartışılacaktır.
