Abstract
Background and Design.- Pseudallöşcheria boydii (Scedosporium apiospermum) and its relative Scedosporium prolificans are hyphomycetes fungi which exist abundantly in soil, manure and polluted water while showing low incidence in outside air although they, are frequent colonizers of the respiratory tract of cystic fibrosis patients. Both fungi can cause difficult to diagnose, therapy-refractory, localized or disseminated infections in both healthy and immunocompromised individuals with high mortality rate. Relevant infections were also reported from Turkey including four maduromycosis (one in hand, three in foot), one bone and soft tissue infection, one arthritis, one lymphadenitis, three eye infections and one skin infection. Three cases were identified at our department since 1981. Recently, a collaborative study project has been prepared to investigate the ecology, transmission and epidemiology, genetic diversity, appropriate diagnostic protocols, data to guide antifungal treatment of these fungi for the development of new strategies for control of pseudaliescheriasis. A study group has been established and supported by European Confederation of Medical Mycology (ECMM). In the present paper we reviewed the cases identified in our department and emphasized the aim and activities of Pseudaliescheriasis Study Group that we participated. Kantarcıoğlu SA, Yücel A. Özet
Pseudallescheria boydii (Scedosporium apiospermum) ve yakını Scedosporium prolificans çevrede toprakta, gübrede ve kirli sularda bol bulunan, havadaki insidansı düşük olan, kistik fibrozluların solunum yollarında kolonizasyon yapabilen, bağışıklığı bozulmuş olan ve olmayan kimselerde bir organ veya sisteme yerleşik veya dissemine, klinik ve laboratuvar tanımı ile tedavisi güç veya olanaksız, mortalitesi yüksek derin mikozlara sebep olabilen, hifli mantarlardır.Bu mantarlardan ileri gelen infeksiyonlara Türkiye'de de rastlanmaktadır. Bu olgular üçü ayakta ve biri elde dört maduromikoz, bir kemik ve yumuşak doku infeksiyonu, bir artrit, bir lenfadenit, üç göz infeksiyonu ve bir deri infeksiyonudur. Anabilim Dalımızda 1981! den bu yana üç olgu tanımlanmıştır. Mantarın ekolojisinin, başta bulaşma yolları olmak üzere epidemiyolojisinin, genetik çeşitliliğinin, uygun tanım yöntemlerinin, tedaviye yol gösterecek antifungal duyarlılık verilerinin araştırılarak bu mikozun kontrol ve tedavisi için yeni stratejiler geliştirilmesini amaçlayan ortak bir proje hazırlanmış ve Avrupa Tıp Mikolojisi Konfederasyonu (ECMM) tarafından desteklenen bir çalışma grubu oluşturulmuştur. Bu yazıda Anabilim Dalımızda tanımlanmış pseudallescheriasis olguları gözden geçirilmekte ve katılmakta olduğumuz ECMM Pseudallescheriasis Çalışma Grubunun amaç ve çalışmalarına değinilmektedir.
