Compare of extrapleural regional analgesia and systemic analgesia in muscle sparing thoracotomy and standard thoracotomy in control of postoperative pain

Abstract
In this study, we compare postthoracotomy pain in the extraplevral regional analgesia and systemic analgesia in cases with standard and muscle sparing thoracotomies. 60 cases (between 14 and 68 years with average 43.4 years), of whom 14 were females and 46 males were included. In 30 cases standard thoracotomy was performed, 0.5% bupivacain infiltration was given to extraplevral area of 15 of this cases by a catheter whilst meperidin and metimazol were given to the rest fifteen patients. We applied the same procedure to another 30 cases with the muscle sparing thoracotomy group. Numerical pain scale was used to measure pain degree. Postoperative pain measurement was recorded after weakness and 24th and 48th hour after operation. The pain was found to be significantly less in the muscle sparing thoracotomy and extra pleural analgesia groups than the others. Seromas were seen under the skin in 14 (47%) cases in the muscle sparing thoracotomy group. Hypotension was seen in three cases (10%) whom were given metimazol and meperidin and bradycardy in one (3%) who was given bubivacain. There was no mortality. To control of postthoracotomy pain, muscle sparing thoracotomy and extrapleural regional analgesia were found to be successful than standard thoracotomy and systemic analgesia.

Bu çalışmada, torakotomi sonrası ağrının kontrolündü standart ve kas kesmemizin yapılan torakotomilerde, ekstraplevral bölgesel analjeziklerle sistemik analjeziklerin etkisinin karşılaştırılması amaçlandı. Çalışmaya yaşları 14-68 (ortalama 43.4) arasında 60 olgu dahil edildi. Bunların 14'ü kadın, 46'sı erkekti. Olguların otuzuna standart posterolateral torakotomi yapıldı, bunların 15'ine torakotomi sırasında ekstraplevral bölgeye kdteter yerleştirilerek %0,5 lik bupivakain infiltrasyonu, diğer 15'ine postoperatif meperidin ve metimazol verildi. "Muscle sparing" torakotomi yapılan diğer 30 olguyada 15 'erli iki gruba ayrılarak aynı yöntem uygulandı. Ağrı şiddetini ölçmek için sayısal ağrı skalası kullanıldı. Hasta uyandığında, postoperatif 24. ve 48. saatte ağrı ölçümü yapıldı. "Muscle sparing" torakotomi yapılıp ekstraplevral bölgesel analjezi uygulanan grupta ağrı, diğer gruplara göre anlamlı derecede azdı. "Muscle sparing" torakotomi uygulanan 14 (%47) olguda cilt altında serama, parenteral metimazol ve meperidin verilen 3 (%10) olguda hipotansiyon, bupivakain verilen bir olguda bradikardi gelişti. Mortalité yoktu. Hastanın konforunu ileri derecede düşüren ve postoperatif komplikasyonlara yol açabilen torakotomi sonrası ağrının kontrolü için "Muscle sparing torakotomi" ve ekstraplevral bölgesel analjezi; standart torakotomi ve parenteral analjeziye göre daha etkili bulunmustur.
