Abstract

It is unavoidable to say that for the Ottoman Empire, the 19th century was aperiod of distress regarding not only military and political aspects, but also in terms ofhealth. As it is known, from the perspective of the European nations, the OttomanEmpire was depicted as the sick man of Europe whose inheritance had been desired to be shared among such nations since the first quarter of the century. Unfortunately,the Ottoman Empire faced serious problems related with the health of the societybesides political struggles. When the State got rid of contagious diseases such aschicken pox which it had tried to overcome for centuries, it came across with diseasessuch as cholera, tuberculosis, syphilis and malaria which appeared in the form of greatepidemics in the 19th century. Although the negative effects of these diesases, such asthe decrease of the population, were obvious, their negative effects were perhaps themost disturbing on people who were at the age of conscription. They were consideredto be of great importance among the general population in terms of supporting the state. However, until that time in the Ottoman Empire, health services had been carriedout by foundations and since epidemics were considered the wrath of God, the fightagainst diseases were prevented because this was regarded as a revolt against God.Therefore, this situation caused the Empire to carry out reforms in order to change thegeneral attitude towards diseases and to pass the health system to the control of theState. In this framework, this article investigates the fight against syphilis which was anepidemic that spread in the whole Kastamonu Province in the second half of the 19thcentury. In this respect, it strikes our attention that particularly at this time hospitalswere built, travelling health delegates were given duty, and constitutions as well asregulations were implemented.

Osmanlı Devleti için 19. Yüzyılın, askeri ve siyasi alanlarda olduğu gibi sağlıkalanında da hayli zor bir süreç olduğunu söylememiz kaçınılmazdır. Bilindiği üzereOsmanlı Devleti, Avrupa devletleri nezdinde yüzyılın ilk çeyreğinden itibarenkendilerince mirası paylaşılmak istenen Avrupanın hasta adamı olarak dilegetiriliyordu. Fakat ne yazık ki siyaseten olduğu gibi toplum sağlığı bakımından daOsmanlı devleti ciddi sorunlarla karşı karşıya kalmıştı. Devlet yüzyıllardır baş et meyeçalıştığı veba ve çiçek gibi bulaşıcı ve salgın hastalıklardan nispeten kurtulmuşkenkarşısına 19. yüzyılda büyük salgınlar halinde kolera, verem, frengi ve sıtma gibihastalıklar çıktı. Bunların nüfusun azalmasında, durağanlaşmasında olumsuz etkileri ortadayken genel nüfusun içinde devletin en önemli dayanağı olan askerlik çağındakifertler üzerindeki olumsuz etkileri belki de en rahatsız edici olanıydı. Ancak o tarihekadar Osmanlıda sağlık hizmetlerinin vakıflar tarafından yürütülüyor olması vesalgınların Allahın gazabı kabul edilmesi hastalıklara karşı mücadeleyi engelliyordu.İşte bu durum devleti, hastalıklara karşı genel bakışı değiştirecek ve sağlık sisteminidevlet kontrolüne geçirecek reformlar yapmaya yöneltti. İşte bu çerçevede olmak üzeremakalede 19. Yüzyılın ikinci yarısında Kastamonu Vilayetinin tamamında salgın halinialan frengi hastalığına karşı yürütülen mücadele konu edilmektedir. Bu husustabilhassa tesis edilen hastaneler, seyyar sıhhiye heyetleri, teşkilat ve yürürlüğe konannizamname ve talimatnameler dikkatimizi çekmiştir.
